摘要:
目的:观察双源CT双能扫描对胃癌病灶及淋巴结的动脉期碘浓度及Overlay值,分析其对胃癌组织学分级的鉴别效能。方法:收集行双源CT扫描的胃癌患者资料,根据术后病理分化类型,将患者分为分化良好组和分化不良组。观察两组间实体病灶及转移淋巴结的碘浓度及Overlay值,通过绘制受试者工作曲线(ROC曲线),评价碘浓度及Overlay值对胃癌组织学分级的鉴别价值。结果:分化不良组胃癌病灶及淋巴结动脉期碘浓度、Overlay值均显著高于分化良好组(均P<0.05)。以病灶动脉期碘浓度及Overlay值诊断胃癌组织学分级的AUC分别为0.798、0.740,以碘浓度1.38 mg/ml为临界值,其敏感度为77.81%,特异度为74.52%,Youden指数为0.523; 以Overlay值33.11 HU为临界值,其敏感度为74.12%,特异度为69.12%,Youden指数为0.432。以淋巴结动脉期碘浓度及Overlay值诊断胃癌组织学分级的AUC为0.784、0.726,以碘浓度1.78 mg/ml为临界值,敏感度为70.41%,特异度为78.21%,Youden指数为0.486; 以Overlay值29.82 HU为临界值,敏感度为77.82%,特异度为64.62%,Youden指数为0.424。结论:双源CT双能扫描动脉期碘浓度及Overlay值对于伴淋巴结转移的胃癌组织学分级有较高的鉴别价值,可为术前评估其组织学分级提供参考依据。
Abstract:
Objective:To observe the iodine concentration and Overlay value of arterial phase in gastric cancer lesions and lymph nodes scanned by dual-source CT,and to analyze the differential efficacy of dual-source CT in histological grading of gastric cancer.Methods:The data of gastric cancer patients who underwent dual-source CT scanning were collected,and the patients were divided into well-differentiated group and poorly-differentiated group according to the type of pathological differentiation after surgery.The iodine concentration and Overlay value of solid lesions and metastatic lymph nodes in the two groups were observed,and the receiver operating curve(ROC curve)was drawn to evaluate the value of iodine concentration and Overlay value in distinguishing the histological grade of gastric cancer.Results:The iodine concentration and Overlay value in the arterial phase of gastric cancer lesions and lymph nodes in the poorly differentiated group were significantly higher than those in the well-differentiated group(all P<0.05).The AUC of histological grading of gastric cancer was 0.798 and 0.740 based on the iodine concentration in arterial phase and the Overlay value,respectively.With the iodine concentration of 1.38 mg/ml as the cut-off value,the sensitivity,specificity and Youden index were 77.81%,74.52% and 0.523,respectively.Taking the Overlay value 33.11 HU as the cut-off value,the sensitivity,specificity and Youden index were 74.12%,69.12% and 0.432,respectively.The AUC of histological classification of gastric cancer was 0.784 and 0.726 based on the iodine concentration and Overlay value of lymph node artery stage,and the sensitivity,specificity and Youden index were 70.41%,78.21% and 0.486 based on the iodine concentration of 1.78 mg/ml as the cut-off value.Taking Overlay 29.82 HU as the cut-off value,the sensitivity,specificity and Youden index were 77.82%,64.62% and 0.424,respectively.Conclusion:The iodine concentration and Overlay value in arterial phase scanned by dual-source CT has a high differential value for histological grade of gastric cancer with lymph node metastasis,and can provide a reference for preoperative evaluation of histological grade.
