摘要:
目的:分析青春期儿童生长发育迟缓的临床特征及独立影响因素。方法:选取116例生长发育迟缓儿童作为发育迟缓组,另选取同龄阶段身高发育正常的儿童80例作为对照组。比较两组一般临床资料、甲状腺功能水平、生长激素(GH)水平,血清矿物质、维生素、叶酸、铁蛋白等营养物质水平,以及炎症因子水平。采用Logistic回归分析青春期儿童生长发育迟缓的独立影响因素。结果:发育迟缓组GH、锌、钙、镁、维生素A、维生素D、维生素B12及叶酸水平明显低于对照组,血清C反应蛋白(CRP)和降钙素原(PCT)水平、挑食比例及每月急性胃肠炎次数明显高于对照组(均P<0.05)。多因素Logistic回归分析显示,GH、钙、维生素D、叶酸、CRP、PCT、挑食、急性胃肠炎是青春期儿童生长发育迟缓的独立影响因素(均P<0.05)。结论:发育迟缓青春期儿童存在明显的GH、矿物质、维生素、叶酸等营养物质缺乏,同时存在明显挑食行为和较差胃肠道健康,应引起临床及家长高度重视。
Abstract:
Objective:To explore the clinical characteristics and independent influencing factors of stunted growth in adolescent children.Methods:A total of 116 children with stunted growth were selected as the stunting group,and 80 children with normal height and development at the same age were selected as the control group.The general clinical data,thyroid function levels,growth hormone(GH)level,serum minerals,vitamins,folic acid,ferritin and other nutrients levels,and the levels of inflammatory factors were compared between the two groups.Logistic regression was used to analyze the independent influencing factors of stunted growth in adolescent children.Results:The levels of GH,zinc,calcium,magnesium,vitamin A,vitamin D,vitamin B12 and folic acid in the stunting group were significantly lower than those in the control group,and the serum CRP and PCT levels,picky eaters ratio and number of monthly acute gastroenteritis were significantly higher than those in the control group(all P<0.05).Multivariate Logistic regression analysis showed that GH,calcium,vitamin D,folic acid,CRP,PCT,picky eaters,and acute gastroenteritis were independent influencing factors of stunted growth in adolescent children(all P<0.05).Conclusion:Adolescent children with stunted growth have obvious deficiencies of GH,minerals,vitamins,folic acid and other nutrients,as well as obvious picky eating behaviors and poor gastrointestinal health.This should arouse the attention of clinicians and parents.
