摘要:
:探讨联合心电图和超声心动图检测对非瓣膜性房颤患者左房室血栓的检出意义。方法:选取非瓣膜性心房颤动(NVAF)患者131例,行食管超声心动图(TEE)检查,并根据检查结果的有无血栓(前)状态将其分为血栓阳性组(38例)、血栓前状态阳性组(35例)和血栓阴性组(58例)。记录所有患者的心电图V1长导联,取20个顺序测量f波振幅平均值,并使用TEE测定左心室舒张末期内径(LVEDD)、左心房内径(LAD)和左心室射血分数(LVEF),对以上参数进行对比分析。结果:三组患者细颤发生率比较差异无统计学意义(均P>0.05)。血栓阳性组患者f波振幅平均值显著低于血栓前状态阳性组和血栓阴性组患者的f波振幅平均值,差异有统计学意义(均P<0.05)。三组患者LVEDD和LVEF比较差异无统计学意义(均P>0.05),而血栓阳性组患者LAD水平高于血栓前状态阳性组和血栓阴性组患者LAD水平,差异有统计学意义(均P<0.05)。结论:使用心电图对f波振幅进行检测对预测NVAF患者血栓栓塞有一定临床价值,联合超声心动图相关检查指标能够提高对血栓栓塞风险的预测。
Abstract:
Objective:To explore the clinical value of electrocardiogram(ECG)combined with echocardiography in the diagnosis of left atrial thrombus in patients with non valvular atrial fibrillation(NVAF).Methods:131 patients with NVAF were selected for esophageal echocardiography(TEE),and they were divided into positive thrombosis group(38 cases),positive pre-thrombosis positive group(35 cases)and negative thrombosis group(58 cases)according to the thrombosis(front)status of the TEE results.The ECG V1 long lead was recorded in all patients,and the average amplitude of f wave was measured in 20 order.The left ventricular end diastolic diameter(LVEDD),left atrium diameter(LAD)and left ventricular ejection fraction(LVEF)were measured with TEE,and the above parameters were compared and analyzed.Results:There was no significant difference in the incidence of fibrillation among the three groups(all P>0.05).The mean f-wave amplitude of the positive thrombosis group was significantly lower than that in the positive pre-thrombosis group and the negative thrombosis group(all P<0.05).There was no statistical significance in LVEDD and LVEF among the three groups(all P>0.05).The level of LAD in positive thrombus group was higher than that in positive pre-thrombosis group and negative thrombosis group(all P<0.05).Conclusion:The detection of f wave amplitude by ECG has certain clinical value in predicting thromboembolism in NVAF patients,and the combination of echocardiographic examination can improve the prediction of thromboembolic risk.
