摘要:
目的:探讨不同手术时机肋骨环抱器手术对肋骨骨折大鼠预后的影响。方法:将建模成功的肋骨骨折大鼠(n=48)随机平分为模型组、早期组与晚期组,早期组与晚期组分别在建模后1 d与3 d后进行环抱器手术治疗,模型组不进行环抱器手术。结果:早期组与晚期组的术后第1周、第2周骨小梁宽度、骨小梁成骨指数都高于模型组(均P<0.05),早期组高于晚期组(P<0.05)。早期组与晚期组的术后第1周、第2周的血清核因子kappaB受体活化因子配体(RANKL)含量低于模型组(均P<0.05),血清骨保护素(OPG)含量高于模型组(均P<0.05),早期组与晚期组对比差异有统计学意义(P<0.05)。早期组与晚期组的术后第1周、第2周的血浆血管内皮生长因子(VEGF)与骨形态发生蛋白-2(BMP-2)含量高于模型组(均P<0.05),早期组高于晚期组(均P<0.05)。早期组与晚期组的术后第1周、第2周的CD4+T淋巴细胞比例高于模型组(P<0.05),早期组高于对照组(P<0.05),三组不同时间点的CD8+T淋巴细胞比例对比差异无统计学意义(均P>0.05)。结论:早期环抱器手术在肋骨骨折大鼠的应用能改善机体免疫反应,促进VEGF、BMP-2、OPG的释放,抑制RANKL的表达,从而促进骨折愈合。
Abstract:
Objective:To compare the effect of rib embracing device surgery at different surgical timing on the prognosis of rib fracture rats.Methods:The successfully modeled rib fracture rats(n=48)were equally randomly divided into model group,early group and late group.The early group and the late group were treated with embracing device surgery 1 and 3 days after modeling,and the model group was not treated embracing device surgery.Results:The trabecular bone width and trabecular osteogenesis index of the early group and the late group in the first and second week after surgery were higher than those of the model group,and the early group were higher than the late group(all P<0.05).The serum RANKL levels of the early group and the late group in the first and second week after surgery were lower than that of the model group,and the serum OPG content was higher than that of the model group,and there was statistically significant difference between the early group and the late group(all P<0.05).The plasma VEGF and BMP-2 levels of the early group and the late group in the first and second week after surgery were higher than those of the model group,and the early group were higher than the late group(all P<0.05).The ratio of CD4+T lymphocytes of the early group and the late group in the first and second week after surgery was higher than that of the model group,and the early group was higher than the late group(all P<0.05).There was no statistically significant difference in the ratio of CD8+T lymphocytes between the three groups at different time points(all P>0.05).Conclusion:The application of early embracing device surgery for rib fracture rats can improve the body's immune response,promote the release of VEGF,BMP-2,OPG and inhibit the expression of RANKL,thereby promote fracture healing.
