摘要:
目的:探讨阿加曲班联合阿司匹林治疗急性脑梗死的疗效及对患者血管内皮功能的影响。方法:急性脑梗死患者86例,以随机数字表法分为试验组(43例)和对照组(43例)。在常规治疗的基础上对照组给予阿司匹林片治疗,试验组在此基础上给予阿加曲班注射液治疗,两组均治疗1周。比较两组治疗后的临床评分、血管内皮功能及血清细胞因子水平; 统计两组治疗期间的不良反应发生率。结果:与治疗前比较,治疗后,两组美国国立卫生研究院卒中量表(NIHSS)、改良Rankin量表(mRS)评分、血清内皮素-1(ET-1)、脂蛋白相关磷脂A2(Lp-PLA2)、基质金属蛋白酶-9(MMP-9)、肿瘤坏死因子-α(TNF-α)水平均降低,试验组均低于对照组(均P<0.05); 而改良Barthel指数(MBI)、血清一氧化氮(NO)、血流介导的舒张功能(FMD)水平均升高,试验组均高于对照组(均P<0.05)。试验组和对照组不良反应发生率比较无统计学差异(4.65%与11.63%,P>0.05)。结论:阿加曲班联合阿司匹林治疗急性脑梗死可明显降低患者血清Lp-PLA2、MMM-9、TNF-α水平,缓解炎症反应,并可改善患者血管内皮功能,减轻神经功能缺损程度,进而有助于促进疾病转归,提高临床疗效,且安全性良好。
Abstract:
Objective:To investigate the efficacy of agatroban combined with aspirin in the treatment of acute cerebral infarction.Methods:86 patients with acute cerebral infarction were divided into test group(43 cases)and control group(43 cases)by random number table method.On the basis of conventional treatment,the control group was given aspirin tablets,and the test group was given agatroban combined with aspirin.Both groups were treated for 1 week.The clinical scores,vascular endothelial function and serum cytokine levels of the two groups after treatment were compared.The incidence of adverse reactions during the treatment period of the two groups was recorded.Results:After treatment,the scores of NIHSS and mRS,the levels of ET-1,Lp-PLA2,MMP-9 and TNF-α in the two groups decreased,and the test group were lower than the control group(all P<0.05); MBI score,NO and FMD levels in the two groups increased,and the test group were higher than the control group(all P<0.05).There was no statistical difference in the incidence of adverse reactions between the two groups(4.65% vs.11.63%,P>0.05).Conclusion:Agatroban combined with aspirin in the treatment of acute cerebral infarction can significantly reduce the patient's serum Lp-PLA2,MMM-9 and TNF-α levels,relieve inflammation response,improve the patient's vascular endothelial function and reduce the degree of neurological deficits,thereby it help to promote disease outcome and improve clinical efficacy,and has good safety.
