摘要:
目的:探讨妊娠期肝内胆汁淤积症(ICP)患者胆固醇代谢相关基因的表达及意义。方法:选择妊娠期肝内胆汁淤积症患者102例作为病例组,选取同期正规产检并分娩的健康孕妇102例作为对照组,检测两组胆固醇代谢血清学指标与相关基因NOS2与SFTPD表达水平,调查与随访妊娠结局并进行相关性分析。结果:病例组的甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、载脂蛋白B(ApoB)水平均高于对照组(均P<0.05),高密度脂蛋白(HDL-C)、载脂蛋白A(ApoA)水平均低于对照组(均P<0.05)。病例组的NOS2与SFTPD基因相对表达水平均高于对照组(均P<0.05)。病例组的早产、剖宫产、新生儿窒息、产后出血、羊水污染等发生率均高于对照组(均P<0.05)。病例组中,NOS2、SFTPD基因相对表达水平与早产、剖宫产、新生儿窒息、产后出血、羊水污染都存在相关性(均P<0.05)。结论:妊娠期肝内胆汁淤积症患者伴随有胆固醇代谢异常,其可增加妊娠不良结局发生率,NOS2与SFTPD基因表达量也增加,且与妊娠不良结局存在相关性。
Abstract:
Objective:To investigate the expression and significance of cholesterol metabolism-related genes in patients with intrahepatic cholestasis of pregnancy(ICP).Methods:102 cases of patients with intrahepatic cholestasis of pregnancy were selected as the case group,and 102 cases of healthy pregnant women during the same period were selected as the control group.The serum indexes of cholesterol metabolism and the expression levels of related genes-NOS2 and SFTPD in the two groups were detected.The pregnancy outcomes in the two groups were followed-up and the correlation analysis was performed.Results:The levels of triglycerides(TG),total cholesterol(TC),low-density lipoprotein(LDL-C)and apolipoprotein B(ApoB)in the case group were higher than those in the control group,and the levels of high-density lipoprotein(HDL-C)and apolipoprotein A(ApoA)were lower than those in the control group(all P<0.05).The relative expression levels of NOS2 and SFTPD genes in the case group were higher than those in the control group(all P<0.05).The incidence of premature delivery,cesarean section,neonatal asphyxia,postpartum hemorrhage,and amniotic fluid contamination in the case group were higher than those in the control group(all P<0.05).In the case group,the relative expression levels of NOS2 and SFTPD genes were correlated with premature delivery,cesarean section,neonatal asphyxia,postpartum hemorrhage,and amniotic fluid pollution(all P<0.05).Conclusion:Patients with intrahepatic cholestasis of pregnancy are accompanied by abnormal cholesterol metabolism,which can increase the incidence of adverse pregnancy outcomes.NOS2 and SFTPD genes expression are also increased,and there are correlation with adverse pregnancy outcomes.
