摘要:
目的:探讨微创Supercap入路与常规入路半髋关节置换治疗老年高龄股骨颈骨折患者的早期临床疗效。方法:选取高龄老年股骨颈骨折患者,根据入路方式的不同分为微创Supercap入路组(微创组)63例与常规入路组(常规组)63例,对比两种不同手术方式在手术切口长度、术后住院时间、失血量、输血量,术后1周髋关节Harris评分、疼痛视觉模拟评分量表(VAS)评分、直立行走测试(TUG)值、肿瘤坏死因子α(TNF-α)、5-羟色胺(5-HT)、前列腺素E2(PGE2)、并发症发生率等方面的差异。结果:微创组手术切口长度、术后住院时间、失血量,术后1周髋关节Harris评分、VAS评分、TUG值,TNF-α、5-HT、PGE2水平均明显小于常规组(均P<0.05)。输血量、并发症发生率两组比较均无统计学差异(均P>0.05)。结论:微创Supercap入路行半髋关节置换对老年高龄股骨颈骨折患者手术损伤小、失血量少,术后疼痛与炎症程度以及功能恢复等方面均较好。
Abstract:
Objective:To explore the early clinical efficacy of minimally invasive Supercap approach and conventional approach in the treatment of elderly patients with femoral neck fractures.Methods:The elderly patients with femoral neck fractures were selected and divided into the minimally invasive Supercap approach group and the conventional group,with 63 cases in each group.The length of the surgical incision,postoperative hospital stay,blood loss,blood transfusion volume,Harris score of the hip joint 1 week after surgery,VAS score,TUG value,TNF-α,5-HT,PGE2 and complication rate were compared between the two groups.Results:The length of surgical incision,postoperative hospital stay,blood loss,Harris score of the hip joint 1 week after surgery,VAS score,TUG value,TNF-α,5-HT,and PGE2 levels in the minimally invasive Supercap approach group were significantly lower than those in the conventional group(all P<0.05).There was no significant difference in blood transfusion volume and complication rate between the two groups(all P>0.05).Conclusion:The minimally invasive Supercap approach for hemi-hip replacement has less surgical damage and blood loss.The postoperative pain and inflammation,and functional recovery in elderly patients with femoral neck fractures are better.
