摘要:
目的:评估不同治疗方案对血流储备临界值时单纯左前降支中度狭窄患者预后的临床意义。方法: 分析通过冠脉造影确诊冠状动脉粥样硬化心脏病患者,且为单纯左前降支中度狭窄血流储备分数(FFR)值处于0.75~0.80之间的患者73例。根据治疗方案,分为经皮冠状动脉介入治疗(PCI)组(43例)、药物组(30例)。通过评估患者基线资料如血清低密度脂蛋白、血尿酸、C反应蛋白等水平,同时统计患者直径狭窄百分比(DS)、面积狭窄百分比(AS)、参考血管直径(RVD)、最小管腔直径(MLD)以及病变长度(LL)等,同时根据心血管事件发生率、Gensini评分等,评估两种治疗方案临床意义。结果:治疗前,两组间一般资料比较,通过冠脉造影发现,Gensini、DS、AS、RVD、MLD水平,差异无统计学意义(均P>0.05),两组之间具有可比性; 治疗后比较治疗前一般资料水平及DS、AS、RVD、MLD等较前有所好转(均P<0.05),但治疗后两组间差异无统计学意义(均P>0.05); 治疗后1年内,PCI组相比药物组发生不良事件发生率更高(P<0.05)。结论: PCI与药物治疗在短时间内心血管发生事件、严重程度改善方面,无明显差异:相比PCI治疗,药物组更安全。
Abstract:
Objective:To evaluate the clinical significance of different treatment schemes in the prognosis of patients with moderate stenosis of left anterior descending artery.Methods:73 patients with coronary atherosclerotic heart disease diagnosed by coronary angiography were analyzed,and the FFR value of moderate stenosis of left anterior descending artery was 0.75-0.80.According to the treatment plan,the patients were divided into PCI group(43 cases)and drug group(30 cases).According to the baseline data of patients such as serum LDL,serum uric acid,C-reactive protein,the percentage of diameter stenosis(DS),the percentage of area stenosis(AS),the reference vessel diameter(RVD),the minimum lumen diameter(MLD)and the length of lesions(LL)were calculated.At the same time,according to the incidence of cardiovascular events and the Gensini score,the clinical significance of the two treatment methods were evaluated.Results:Before treatment,there was no significant difference in general data,Gensini score,DS,AS,RVD and MLD between the two groups(all P>0.05).After treatment,the general data level and DS,AS,RVD and MLD were improved,but there was no significant difference between the two groups(all P>0.05).Within 1 year after treatment,the incidence of adverse events in PCI group was higher than that in drug group(P<0.05).Conclusion:There was no significant difference in short-term cardiovascular events and severity improvement between PCI and drug therapy.Compared with PCI,drug group was safer.
