摘要:
目的:探究甘露醇联合肠内营养治疗脑出血疗效及对患者免疫功能的影响。方法:选取脑出血患者110例,随机分为药物治疗组、联合治疗组各55例,两组患者分别给与相应的治疗。对两组患者进行NIHSS、Barthel、MMSE评分,酶联免疫吸附实验法检测血清白细胞介素-4(IL-4)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平,流式细胞仪检测T淋巴细胞亚群水平,使用全自动生化分析仪对血红蛋白(Hb)、血清总蛋白(TP)、白蛋白(ALB)水平进行检测,对比两组治疗效果、不良反应。结果:治疗后,与药物治疗组比较,联合治疗组NIHSS评分较低,Barthel、MMSE评分较高(均P<0.05)。治疗后联合治疗组患者血清IL-4、hs-CRP、TNF-α、CD8+水平均低于药物治疗组,CD4+、CD3+、Hb、TP、ALB水平均高于药物治疗组(均P<0.05)。联合治疗组治疗总有效率高于药物治疗组(P<0.05)。结论:脑出血患者使用肠内营养联合甘露醇治疗,神经功能受损情况减轻,认知功能、日常生活能力、营养状况、免疫功能明显改善,治疗效果显著。
Abstract:
Objective:To explore the efficacy of mannitol combined with enteral nutrition in the treatment of cerebral hemorrhage and its influence on patients' immune function.Methods:110 patients with cerebral hemorrhage were selected and randomly divided into drug treatment group and combined treatment group,with 55 patients in each group.The two groups were given corresponding treatments.NIHSS,Barthel and MMSE scores were performed for the two groups.The levels of serum interleukin-4(IL-4),hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)were detected by enzyme-linked immunosorbent assay.The levels of T lymphocyte subsets were detected by flow cytometry.The levels of hemoglobin(Hb),total serum protein(TP),albumin(ALB)were measured by automatic biochemical analyzer.The treatment effect and adverse reactions were compared between the two groups.Results:After treatment,compared with the drug treatment group,the combined treatment group had lower NIHSS score and higher Barthel and MMSE scores(all P<0.05).After treatment,the serum levels of IL-4,hs-CRP,TNF-α and CD8+ in the combined treatment group were lower than those in the drug treatment group,and the levels of CD4+,CD3+,Hb,TP and ALB were higher than those in the drug treatment group(all P<0.05).The total effective rate of combined treatment group was higher than that of drug treatment group(P<0.05).Conclusion:Mannitol combined with enteral nutrition in the treatment of patients with cerebral hemorrhage can reduce neurological impairment,improve cognitive function,ability of daily living,nutritional status,and immune function,and have significant therapeutic effect.
