摘要:
目的:探讨叶酸、维生素E联合硫酸镁治疗妊娠期高血压患者疗效及对患者血管内皮功能、Th1/Th2细胞水平的影响。方法:选取妊娠期高血压患者210例为研究对象进行前瞻性随机对照研究,以随机数字表分为联合组(n=105)、对照组(n=105)。在常规治疗基础上,对照组予以硫酸镁治疗,联合组予以叶酸、维生素E联合硫酸镁,均治疗2周。对比两组临床疗效、妊娠结局及治疗前后舒张压(DBP)、收缩压(SBP)、24 h尿蛋白定量、血清血管内皮功能指标[一氧化氮(NO)、内皮素-1(ET-1)]、氧化应激指标[丙二醛(MDA)、髓过氧化物酶(MPO)、缺血修饰白蛋白(IMA)]、Th1、Th2、Th1/Th2细胞水平。结果:①疗效及妊娠结局:联合组总有效率高于对照组,剖宫产、早产、胎儿宫内窘迫、新生儿低体重、新生儿窒息发生率均低于对照组(均P<0.05),两组产后出血发生率相比,差异无统计学意义(P>0.05); ②血压与尿蛋白:治疗后两组DBP、SBP水平、24 h尿蛋白定量均明显降低,联合组降低幅度高于对照组(P<0.05); ③血管内皮功能:两组治疗后血清NO水平较治疗前提高,ET-1水平较治疗前下降,且联合组均优于对照组(P<0.05); ④氧化应激:治疗后,联合组血清MDA、MPO、IMA水平均低于对照组(均P<0.05); ⑤Th1/Th2细胞:与治疗前比较,治疗后两组血清Th1、Th1/Th2水平降低,Th2水平升高,且联合组均优于对照组(均P<0.05)。结论:叶酸、维生素E联合硫酸镁治疗妊娠期高血压临床疗效显著,可有效控制患者血压,缓解机体氧化应激状态,抑制肾脏及血管内皮损伤,同时调节Th1/Th2细胞平衡,改善妊娠结局。
Abstract:
Objective:To investigate the Efficacy of folic acid,vitamin E combined with magnesium sulfate in the treatment of gestational hypertension and its effect on vascular endothelial function and Th1/Th2 cell level.Methods:210 patients with gestational hypertension were selected as the research subjects for prospective randomized controlled study.They were divided into combined group(n=105)and control group(n=105)by random number table.On the basis of conventional treatment,the control group was given magnesium sulfate,and the combined group was given folic acid and vitamin E to assist magnesium sulfate,all of which were treated for 2 weeks.The efficacy,pregnancy outcome and diastolic blood pressure(DBP),systolic blood pressure(SBP),24-hour urine protein quantitation,and serum vascular endothelial function indexes [nitrogen oxide(NO),endothelin-1(ET-1)],and oxidative stress indexes [malondialdehyde(MDA),myeloperoxidase(MPO),ischemia-modified albumin(IMA)],Th1/Th2 cells(Th1,Th2,Th1/Th2)levels of the two groups were compared.Results:The total effective rate of the combined group was higher than that of the control group,and the incidence of cesarean section,premature delivery,intrauterine distress,low birth weight of neonates,and neonatal asphyxia was lower than that of the control group(all P<0.05),and there was no statistically significant difference in the incidence of postpartum hemorrhage between the two groups(P>0.05).After treatment,the levels of DBP and SBP and the urinary protein quantification in 24 hours were significantly reduced,and the reduction in the combined group was higher(P<0.05).After treatment,serum NO level in the two groups was higher than that before treatment,and ET-1 level was lower than that before treatment,and the combined group was superior to the control group(P<0.05).After treatment,the serum MDA,MPO,IMA levels of the combined group were lower than those of the control group(all P<0.05).The serum Th1,Th1/Th2 levels decreased and Th2 levels increased in both groups after treatment,and the combined group was superior to the control group(all P<0.05).Conclusion:Folic acid,vitamin E combined with magnesium sulfate has a significant clinical effect in the treatment of gestational hypertension,which can effectively control the patient's blood pressure,relieve the body's oxidative stress state,inhibit kidney and vascular endothelial damage,regulate Th1/Th2 cell balance and improve pregnancy outcomes.
