摘要:
目的:研究实时剪切波弹性成像(SWE)联合甲状腺球蛋白抗体(Tg-Ab)检测在甲状腺癌诊断中的应用价值。方法:将甲状腺癌患者98例纳入研究,记作甲状腺癌组,另取甲状腺良性疾病患者98例作为良性疾病组; 对所有受试者均进行SWE检查,且以病理检查为金标准,分析SWE检查诊断甲状腺癌的效能; 采集所有受试者的清晨空腹静脉血,获取血清以酶联免疫吸附法检查Tg-Ab水平并对比; 以受试者工作特征(ROC)曲线分析SWE结合血清Tg-Ab水平诊断甲状腺癌的效能; 分析血清Tg-Ab水平与甲状腺癌患者临床病理特征的关系; 比较不同病变患者的血清半乳糖凝集素-1(Gal-1)以及谷胱甘肽过氧化物酶3(GPX3)水平。结果:以病理诊断为金标准,SWE诊断甲状腺癌的灵敏度为83.67%,特异度为78.57%,准确度为81.12%; 甲状腺癌组血清Tg-Ab水平为(120.49±45.82)U/ml,高于良性疾病组的(72.29±31.95)U/ml,差异有统计学意义(t=8.542,P=0.000)。SWE结合血清Tg-Ab水平诊断甲状腺癌的曲线下面积、敏感度以及特异度均高于SWE、血清Tg-Ab水平单独诊断。临床分期为Ⅲ-Ⅳ期的甲状腺患者血清Tg-Ab水平高于Ⅰ-Ⅱ期患者(P<0.05); 而年龄、性别、病灶直径以及淋巴结转移均与血清Tg-Ab水平无关(均P>0.05); 甲状腺癌组血清Gal-1水平高于良性疾病组,而GPX3水平低于良性疾病组(均P<0.05)。结论:SWE结合Tg-Ab检测应用于甲状腺癌诊断中的效能较高,且血清Tg-Ab水平和甲状腺癌患者临床分期密切相关。
Abstract:
Objective:To study the application value of real-time shear wave elastography(SWE)combined with the detection of thyroid globulin antibody(Tg-Ab)in the diagnosis of thyroid cancer.Methods:A total of 98 patients with thyroid cancer were included in the study and recorded as the thyroid cancer group.Another 98 patients with benign thyroid disease at the same time were taken as the benign disease group.SWE examination was performed on all subjects,and pathological examination was used as the gold standard to analyze the efficacy of SWE examination in diagnosing thyroid cancer.In addition,fasting venous blood of all subjects was collected in the morning,and serum was obtained.Tg-Ab levels were examined by enzyme-linked immunosorbent assay and compared.ROC curve was used to analyze the efficacy of SWE combined with serum Tg-Ab level in the diagnosis of thyroid cancer.The relationship between serum Tg-Ab level and clinicopathological characteristics of patients with thyroid cancer was analyzed.Serum levels of galactose agglutinin -1(Gal-1)and glutathione peroxidase 3(GPX3)were compared in patients with different lesions.Results:The sensitivity,specificity and accuracy of SWE in the diagnosis of thyroid cancer were 83.67%,78.57%,81.12%,respectively.Serum Tg-Ab level in the thyroid cancer group was(120.49±45.82)U/ml,which was higher than that in the benign disease group [(72.29±31.95)U/ml],and the difference was statistically significant(t=8.542,P=0.000).The area under the curve,sensitivity and specificity of SWE combined with serum Tg-Ab in the diagnosis of thyroid cancer were higher than SWE and serum Tg-Ab level alone.Serum Tg-Ab level in patients with clinical stage Ⅲ-Ⅳ was higher than that in patients with clinical stageⅠ-Ⅱ(P<0.05).However,age,gender,lesion diameter and lymph node metastasis were not correlated with serum Tg-Ab level(all P>0.05).The serum Gal-1 level of thyroid cancer group was higher than that of benign disease group,while GPX3 level was lower than that of benign disease group(all P<0.05).Conclusion:The efficacy of SWE combined with Tg-Ab in the diagnosis of thyroid cancer is high,and the serum Tg-Ab level is closely related to the clinical stage of thyroid cancer patients.
