摘要:
目的:探讨妊娠高血压早期肾损伤情况及其与血清视黄醇结合蛋白(sRBP)、尿酸(UA)的关系。方法:选取妊娠高血压孕妇85例作为疾病组,根据其严重程度,将孕妇分为妊娠高血压组(n=30)、子痫前期组(n=38)、重度子痫前期组(n=17),另选取同期体检的健康孕妇83例作为正常组,比较各组血清sRBP、UA水平; 根据妊娠高血压孕妇是否合并肾损伤,将其分为肾损伤组(n=40)、非肾损伤组(n=45),分析血清sRBP、UA水平与妊娠高血压早期肾功能损伤的关系。结果:①疾病组孕妇血清sRBP、UA水平分别为(86.82±9.64)mg/L、(276.28±49.78)μmol/L显著高于正常组孕妇的(42.34±5.49)mg/L、(258.14±45.23)μmol/L(均P<0.05); ②妊娠高血压组孕妇血清sRBP、UA水平分别为(84.57±5.27)mg/L、(268.95±20.08)μmol/L显著低于子痫前期组(87.01±6.33)mg/L、(276.57±28.58)μmol/L(均P<0.05); ③妊娠高血压孕妇合并肾损伤组血清sRBP、UA分别为(89.27±5.84)mg/L、(281.85±19.63)μmol/L高于非肾损伤组孕妇的(84.66±4.79)mg/L、(271.33±27.08)μmol/L(均P<0.05); ④ROC曲线分析显示,血清sRBP、UA联合预测孕妇肾功能损伤的AUC、特异度、敏感度为0.878、80.2%、85.6%。 结论:妊娠高血压孕妇血清sRBP、UA出现升高,两者联合检测可预测早期肾损伤的发生。
Abstract:
Objective:To explore the early renal injury in gestational hypertension and its relationship with serum retinol binding protein(sRBP)and uric acid(UA).Methods:A total of 85 pregnant women with gestational hypertension were enrolled as disease group.According to the severity of gestational hypertension,they were divided into gestational hypertension group(n=30),pre-eclampsia group(n=38)and severe pre-eclampsia group(n=17).Another 83 healthy pregnant women who underwent physical examination during the same period were enrolled as normal group.The levels of serum sRBP and UA among all groups were compared.According to presence or absence of renal injury,the pregnant women with gestational hypertension were divided into renal injury group(n=40)and non-renal injury group(n=45).The relationship between serum sRBP,UA levels and early renal function injury was analyzed.Results: The levels of serum sRBP and UA in disease group were(86.82±9.64)mg/L and(276.28±49.78)μmol/L,which were significantly higher than those in normal group [(42.34±5.49)mg/L,(258.14±45.23)μmol/L](all P<0.05).The levels of serum sRBP and UA in gestational hypertension group were(84.57±5.27)mg/L and(268.95±20.08)μmol/L,which were significantly lower than those in severe pre-eclampsia group [(90.38±3.08))mg/L,(288.56±19.37)μmol/L](all P<0.05).The levels of serum sRBP and UA in renal injury group were(89.27±5.84)mg/L and(281.85±19.63)μmol/L,which were higher than those in non-renal injury group [(84.66±4.79)mg/L,(271.33±27.08)μmol/L](all P<0.05).ROC curve analysis showed that AUC,specificity and sensitivity of serum sRBP combined with UA for predicting renal function injury were 0.878,80.2% and 85.6%,respectively.Conclusion:The levels of serum sRBP and UA are increased in pregnant women with gestational hypertension.The combination of the two can provide more accurate prediction for the occurrence of early renal injury.
