摘要:
目的:探讨胱抑素C、甘油三酯、D-二聚体、纤维蛋白原在糖尿病肾病(DN)早期诊断与肾损伤程度评估中的价值。方法:将292例糖尿病肾病患者分为代偿期肌酐正常组、代偿期肌酐升高组、失代偿期组、肾衰竭期组和尿毒症期组。检测各组胱抑素C、甘油三酯、D-二聚体和纤维蛋白原水平并进行组间比较。结果:五组患者胱抑素C、D-二聚体、纤维蛋白原检测结果有逐渐增高趋势,多组间比较差异均有统计学意义(均P<0.05); 甘油三酯多组间比较,差异无统计学意义(P>0.05)。代偿期肌酐正常组中胱抑素C、D-二聚体、纤维蛋白原阳性率分别为79.3%、27.6%、24.1%,均低于其他各组的水平,差异有统计学意义(P<0.05); 甘油三酯阳性率(62.1%)高于其他各组的水平,差异有统计学意义(P<0.05); 胱抑素C阳性率(79.3%)高于同组中D-二聚体(27.6%)和纤维蛋白原阳性率(24.1%),差异有统计学意义(P<0.05)。结论:胱抑素C和甘油三酯对DN早期诊断有一定价值,并且胱抑素C、D-二聚体和纤维蛋白原能够作为DN肾脏损害程度的参考指标。
Abstract:
Objective:To investigate the value of cystatin C,triglyceride,D-dimer and fibrinogen in early diagnosis and evaluation of renal injury in diabetic nephropathy(DN).Methods:292 patients with diabetic nephropathy were divided into normal compensatory creatinine group,elevated creatinine group,decompensated phase group,renal failure stage group and uremic stage group.The levels of cystatin C,triglyceride,D-dimer and fibrinogen were detected and compared among groups.Results:The detection results of cystatin C,D-dimer and fibrinogen in the five groups showed a trend of increasing gradually,and the differences among the groups were statistically significant(all P<0.05).There was no significant difference in triglyceride among multiple groups(P>0.05).The positive rates of cystatin C,D-dimer and fibrinogen in normal compensatory creatinine group were 79.3%,27.6% and 24.1%,respectively,which were lower than those in other groups(P<0.05).The positive rate of triglyceride(62.1%)was higher than that of other groups(P<0.05).The positive rate of cystatin C(79.3%)was higher than that of D-dimer(27.6%)and fibrinogen(24.1%)in the same group(P<0.05).Conclusion:Cystatin C and triglyceride have certain value in the early diagnosis of DN,and cystatin C,D-dimer and fibrinogen can be used as reference indicators of kidney damage in DN.
