摘要:
目的:探讨复方米诺环素奥硝唑温敏凝胶治疗老年慢性牙周炎的疗效及对龈沟液中人类刺猬因子(Shh)蛋白、基质金属蛋白酶-8(MMP-8)水平的影响。方法:选择100例老年慢性牙周炎患者,按照随机数表法,分为研究组和对照组各50例,对照组在常规牙周基础治疗上给予米诺环素治疗,研究组在常规牙周基础治疗上给予复方米诺环素奥硝唑温敏凝胶治疗,均持续治疗4周。比较两组患者治疗4周后的临床疗效、治疗前及治疗4周后牙周袋深度(PD)、牙菌斑指数(PLI)、龈沟出血指数(SBI)、牙龈指数(GI)、龈沟液中Shh蛋白、MMP-8水平及治疗期间的不良反应。结果:治疗4周后,研究组的总有效率为96.00%,对照组为82.00%,研究组明显高于对照组,差异有统计学意义(P<0.05); 治疗4周后,两组患者PD、PLI、SBI、GI、龈沟液中Shh蛋白、MMP-8水平均明显降低,且研究组低于对照组,差异有统计学意义(P<0.05); 治疗期间,两组不良反应情况比较,差异无统计学意义(P>0.05)。结论:复方米诺环素奥硝唑温敏凝胶治疗老年慢性牙周炎的疗效显著,可有效改善牙周指标,降低龈沟液中Shh蛋白、MMP-8水平的表达。
Abstract:
Objective:To study the efficacy of compound minocycline ornidazole thermosensitive gel in the treatment of elderly chronic periodontitis and its effect on sonichedgehog(Shh)protein and matrix metalloproteinase-8(MMP-8)levels in gingival crevicular fluid.Methods:100 elderly patients with chronic periodontitis were selected and randomly divided into study group and control group by the random number table method,with 50 cases in each group.The control group was treated with minocycline on the basis of routine periodontal treatment,and the study group was given compound minocycline ornidazole thermosensitive gel on the basis of routine periodontal treatment,and lasted for 4 weeks.The clinical efficacy after 4 weeks of treatment,the changes of the periodontal pocket depth(PD),plaque index(PLI),gingival crevicular bleeding index(SBI),gingival index(GI),Shh protein and MMP-8 levels in gingival crevicular fluid before and after 4 weeks of treatment,and adverse reactions during treatment were compared between the two groups.Results:After 4 weeks of treatment,the total effective rate of the study group was 96.00%,that of the control group was 82.00%.The results of the study group were significantly higher than those of the control group(P<0.05).After 4 weeks of treatment,the levels of PD,PLI,SBI,GI,Shh protein and MMP-8 in gingival crevicular fluid of the two groups were significantly lower than those before treatment,and the results of the study group were significantly lower than those of the control group(P<0.05).During the treatment,there was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion:Compound minocycline ornidazole thermosensitive gel is effective in treating chronic periodontitis in elderly patients,which can effectively improve periodontal indexes and reduce the expression of Shh protein and MMP-8 in gingival crevicular fluid.
