摘要:
目的:探讨超声引导下微创消融治疗子宫腺肌症的安全性及有效性。方法:回顾性分析178例行超声引导下微创消融治疗子宫腺肌症患者的临床资料; 超声造影评估子宫病变区消融率及随访期间体积变化(术后1、3、6、12月),对比分析治疗前后实验室资料变化(血红蛋白、糖类抗原CA125)以及相关症状及生活质量评分。结果:术后1 d超声造影提示平均消融率为(91.34±5.42)%; 术后观察1、3、6、12 个月; 病灶体积缩小率、血红蛋白、CA125、相关症状评分较术前差异均有统计学意义(P<0.05); 术后3个月病灶体积缩小最佳(61.16%±18.13%),术后1个月血红蛋白上升最高[(112.34±11.56)g/ml],术后12个月CA125降至最低[(51.74±17.39)IU/ml]; 所有患者均无严重并发症; 治疗患者中,疗效非常显著155例、疗效显著21例、有效1例、无效1例; 局限性患者术后1~2年复发率约在18%(25/139)、弥漫性患者约在38%(15/39)。结论:超声引导下微创消融术治疗子宫腺肌症疗效显著,且安全、微创,具有很高的临床应用价值; 局限性子宫腺肌症远期疗效优于弥漫性子宫腺肌症患者。
Abstract:
Objective:Objective:To investigate the safety and availability of ultrasound-guided minimally invasive ablation in the treatment of adenomyosis.Methods:The clinical data of the 178 patients with adenomyosis treated by ultrasound-guided minimally invasive ablation were retrospectively analyzed.Evaluated the ablation rate and the volume change of uterine lesions with contrast-enhanced ultrasound(1,3,6 and 12 months after ablation).Compared and analysed the laboratory data(hemoglobin,CA125)、UFS-QOL score between the preoperative and postoperative.Results:The average ablation rate was(91.34±5.42)% by contrast-enhanced ultrasound on the second day after the ablation.Follow-up 1,3,6,12 months after the ablation.The difference of lesion volume reduction rate,hemoglobin,CA125,UFS-QOL score was statistically significant compared with that before ablation(P<0.05).The lesion volume reduction rate was best at the third months after the ablation,which was(61.16±18.13)%.The increase of hemoglobind was fastest at the first month after the ablation,which was(112.34±11.56)g/ml.The decrease of CA125 was lowest at the twelfth month after the ablation,which was(51.74±17.39)IU/ml.None of the patients had serious complications.Among the patients,155 had extremely effective,21 had obvious effect,1 had general effect and 1 had no effect.The palindromia rate was 18%(25/139)in localized type and 38%(15/39)in diffuse type at 1-2 years after the ablation.Conclusion:Ultrasound-guided minimally invasive ablation is significantly effective in the treatment of adenomyosis,which is safe and minimally invasive,and has great clinical application value.The long-term efficacy of localized adenomyosis is better than that of diffuse adenomyosis.
