摘要:
目的:探讨脑出血穿刺引流术后灌注尿激酶剂量和时间对患者预后的影响及临床应用价值。方法:选取脑出血患者30例在穿刺引流术后采用50 000 U尿激酶灌注4 h方案为试验组,另外30例穿刺引流术后采用20 000 U尿激酶灌注2 h方案为对照组,对比两组的血肿清除率、血肿清除时间、术后再出血、术后7 d 格拉斯哥昏迷评分(GCS)等方面的差异。结果:与对照组比较,试验组的血肿清除率更高、血肿清除时间更短、灌注次数更少,两组比较差异均有统计学意义; 两组在术后7 d GCS、术后再出血率、颅内感染率、应激性溃疡发生率、病死率、格拉斯哥预后评分(GOS)方面比较差异无统计学意义。结论:穿刺引流血肿清除术后采用50 000 U尿激酶灌注4 h较20 000 U尿激酶灌注2 h对于患者是安全的,治疗效果更佳,更适合在临床中应用。
Abstract:
Objective:To investigate the effect of the dose and time of urokinase perfusion after intracerebral hemorrhage puncture drainage on the prognosis and clinical application value.Methods:After puncture drainage,30 cerebral hemorrhage patients treated with 50000 U urokinase perfusion for 4 hours were selected as the experimental group,and another 30 cerebral hemorrhage patients treated with 20000 U urokinase perfusion for 2 hours as the control group.The hematoma clear rate,hematoma clear time,postoperative rebleeding,GCS score 7 days after surgery and other data were analyzed and compared between the two groups.Results:Compared with the control group,the hematoma clear rate in the experimental group was higher,the hematoma clear time was shorter,and the number of perfusion times was less,and the differences were statistically significant.There were no significant differences in GCS score at 7 days after surgery,postoperative rebleeding,intracranial infection rate,stress ulcer,mortality and GOS score between the two groups.Conclusion:Compared with 20000 U urokinase perfusion for 2 hours after the removal of hematoma by puncture drainage,infusion of 50000 U urokinase for 4 hours is safer and more effective for patients.
