摘要:
目的:分析基于面神经增强曲面重建技术诊断面神经炎患者的临床特点。方法:对收治的86例面神经炎患者进行研究,使用MR增强曲面重建对颅内面神经的变化情况进行研究,比较各受累部位患侧与健侧颅内面神经与同一层面邻近颞叶的信号强度比值(SIR)、患侧与健侧瞬间反射R1、R2、R2'、面神经潜伏期及波幅,并分析面神经增强曲面重建技术在面神经炎诊断中的作用。结果:颅内各受累部位的患侧面神经SIR值均高于健侧,相比较差异有统计学意义(P<0.05)。患侧面神经瞬间反射R1、R2、R2'值均长于健侧,相比较差异有统计学意义(P<0.05)。患侧面神经电图潜伏期长于健侧,波幅短于健侧,差异有统计学意义(P<0.05)。观察面神经炎患者的3D BrainVIEW T1WI序列,显示各受累部位均呈现不同程度的强化。将SIR值不小于最佳截取点的值赋值为1,小于该值赋值为0,显示面神经增强曲面重建技术诊断的AUC为0.987(95%CI为0.981~0.997),预测的敏感性为89.43%,特异度为97.81%,具有较佳的诊断效果。结论:面神经炎患者患侧与健侧的SIR值、瞬间反射R1、R2、R2'值、面神经电图潜伏期及波幅均存在差异,面神经增强曲面重建技术对于面神经炎具有较佳的诊断效果。
Abstract:
Objective:To analyze the clinical features of patients with facial neuritis based on facial nerve enhancement curved planar reformation(CPR)technology.Methods:86 patients with facial neuritis were studied.The changes of intracranial facial nerve were studied by using MR enhanced CPR.The signal intensity ratio(SIR)between the affected side and the healthy side of the intracranial facial nerve and the adjacent temporal lobe at the same level,the instantaneous reflex R1,R2,R2',and the latency and amplitude of the facial nerve were compared.The role of CPR in the diagnosis of facial neuritis was analyzed.Results:The SIR values of the affected side were higher than those of the healthy side,and the differences were statistically significant(P<0.05).The values of instantaneous reflex R1,R2 and R2' in the affected side were longer than those in the healthy side(P<0.05).The latency of the EEG of the affected side was longer than that of the healthy side,and the amplitude was shorter than that of the healthy side(P<0.05).The 3D BrainVIEW T1WI sequence of facial neuritis patients showed different degrees of enhancement in the affected parts.The value of the SIR value not less than the optimal intercept point was assigned to 1,and the value less than this value was assigned to 0.The results showed that the AUC of facial nerve enhanced CPR is 0.987(95%CI 0.981-0.997),the sensitivity of prediction was 89.43%,and the specificity was 97.81%.Conclusion:There are differences in the SIR value,instantaneous reflex R1,R2,R2' value,the latency and amplitude between the affected side and the healthy side.The facial nerve enhancement surface curved planar reformation technology has a better diagnostic effect for facial neuritis.
