摘要:
目的:探讨脑梗死患者血清炎症因子变化及其与颈动脉斑块稳定性、神经功能缺损程度的关系。方法:收集脑梗死患者90例的临床资料进行回顾性分析,将其设为研究组,将同期体检健康者90例设为对照组,按患者颈动脉斑块稳定性不同将研究组分为不稳定组(n=34)与稳定组(n=56),按患者神经功能缺损程度不同将研究组分为重度组(n=48)、非重度组(n=42),观察比较研究组与对照组的血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)的浓度,记录分析脑梗死患者颈动脉斑块稳定性、神经功能缺损程度的相关影响因素,以及脑梗死患者各临床指标与其颈动脉斑块稳定性、神经功能缺损程度的相关性。结果:研究组血清TNF-α、IL-6、CRP浓度均明显高于对照组(P<0.05); 稳定组的血清TNF-α、IL-6、CRP浓度均明显高于不稳定组(P<0.05); 重度组的血清TNF-α、IL-6、CRP浓度均明显高于非重度组(P<0.05); 血清TNF-α、IL-6、CRP浓度是脑梗死患者颈动脉斑块稳定性、神经功能缺损程度的独立影响因素(OR>1,P<0.05)。结论: 脑梗死患者较正常人血清TNF-α、IL-6、CRP浓度明显升高; 血清TNF-α>1.54 ng/ml、IL-6>0.46 ng/L、CRP>5.20 mg/L是脑梗死患者颈动脉斑块不稳定、神经功能重度缺损的独立危险因素。
