摘要:
目的:探讨瑞舒伐他汀与替罗非班联合冠脉介入(PCI)术治疗急性心肌梗死(AMI)效果及对患者心功能、循环内皮祖细胞(EPCs)数量的影响。方法:选取AMI患者130例,均行PCI术,予以瑞舒伐他汀治疗的65例作为对照组,予以瑞舒伐他汀与替罗非班治疗的65例作为观察组。对比两组不良反应发生率、术前、术后1周心功能[左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)]、血清炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)]水平、肱动脉内径、循环EPCs数量、血管内皮细胞功能[一氧化氮(NO)、内皮素-1(ET-1)、血管性假血友病因子(vWF)]水平,随访1个月,对比两组术前、术后1个月生活质量(SAQ评分)。结果:两组术后1周LVEF均升高,且观察组高于对照组,LVEDD、LVESD水平降低,且观察组低于对照组(P<0.05); 两组术后1周血清IL-6、TNF-α、hs-CRP、ET-1、vWF水平下降,且观察组低于对照组,血清NO水平增高,且观察组高于对照组(P<0.05); 两组术后1周肱动脉内径、循环EPCs数量提高,且观察组高于对照组(P<0.05); 随访1个月无脱落病例,观察组SAQ评分高于对照组(P<0.05)。结论:瑞舒伐他汀与替罗非班联合PCI术治疗AMI不仅能有效缓解炎症反应,且具有血管内皮保护作用,能进一步改善心功能及生活质量。
Abstract:
Objective:To investigate the therapeutic effect of rosuvastatin and tirofiban combined with percutaneous coronary intervention(PCI)in the treatment of patients with acute myocardial infarction(AMI)and its influence on the patient's cardiac function and the number of circulating endothelial progenitor cells(EPCs).Methods:A total of 130 patients with AMI were selected.All patients underwent PCI.65 patients treated with rosuvastatin were in the control group,and 65 patients treated with rosuvastatin and tirofiban were in the observation group.The incidence of adverse reactions,cardiac function [left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)],and serum inflammatory factors [Interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP)] levels,internal diameter of the brachial artery,number of circulating EPCs,function of vascular endothelial cells [nitric oxide(NO),endothelin-1(ET-1),vascular pseudohemophilia factor(vWF)] before and one week after operation were compared between the two groups,follow-up was performed for one month,and the quality of life(SAQ score)was compared between the two groups.Results:In both groups at one week after operation,the LVEF increased,and the LVEF of the observation group was higher than that of the control group; the LVEDD and LVESD decreased,and the LVEDD and LVESD of the observation group were lower than those of the control group(P<0.05); the levels of serum IL-6,TNF-α,hs-CRP,ET-1 and vWF decreased,and the levels of serum IL-6,TNF-α,hs-CRP,ET-1 and vWF of the observation group were lower than those of the control group; the serum NO level increased,and the serum NO level of the observation group was higher than that of the control group(P<0.05); the brachial artery inner diameter and the number of circulating EPCs increased,and the brachial artery inner diameter and the number of circulating EPCs of the observation group were higher than those of the control group(P<0.05).There was no case of shedding during the follow-up for one month,and the SAQ score in the observation group was higher than that in the control group(P<0.05).Conclusion:Rosuvastatin and tirofiban combined with PCI for AMI can not only effectively relieve the inflammatory response,but also protect vascular endothelium,which can further improve heart function and quality of life.
