摘要:
目的:利用磁共振(MR)技术分析急性非接触性膝关节前外侧韧带(ALL)损伤与外侧稳定结构损伤的相关性,从而推断ALL是否具有限制膝关节旋转及移位等作用。方法:回顾性分析因急性非接触性膝关节损伤进行MR检查的患者的影像学资料,分类统计膝关节外侧稳定结构各组成部分的显示率; 对ALL损伤与外侧稳定结构损伤进行相关性分析。结果:纳入88例膝关节MR图像,ALL、弓状韧带(AL)以及腘腓韧带(PFL)为非恒定显示结构,显示率分别为77.3%、26.1%及29.5%。髂胫束(ITB)、Kaplan纤维束、髌外侧支持带、腘肌腱(PT)、股二头肌肌腱(BT)及外侧副韧带(LCL)为恒定显示结构,显示率为100%。ALL损伤与ITB、Kaplan纤维束、髌外侧支持带、PT、BT、PFL及LCL损伤具有相关性(P<0.05),与AL损伤不具有相关性(P>0.05)。结论:ALL参与膝关节多种运动形式,不仅是限制膝关节内旋的次级稳定器,而且参与膝关节外旋、胫骨前移及抵抗膝关节的内翻应力。
Abstract:
Objective:To analyze the correlation between the acute non-contact injury of anterolateral ligament(ALL)and the injury of lateral stable structure of the knee joint by MR technique,and to infer whether the anterolateral ligament has the function of limiting the rotation and displacement of the knee joint.Methods:The imaging data of patients with acute non-contact knee injury who underwent MRI examination were analyzed retrospectively,and the display rate of each component of lateral stable structure of knee joint was classified and counted,and the correlation analysis between the injury of ALL and the injury of lateral stable structure was conducted.Results:In 88 cases of MR images of knee joint,the ALL,arcuate ligament(AL)and popliteofibular ligament(PFL)were unsteady display structures,with the display rates of 77.3%,26.1% and 29.5%,respectively.The iliotibial band(ITB),Kaplan fiber bundle,lateral patellar retinaculum,popliteal tendon(PT),biceps femoral tendon(BT)and lateral collateral ligament(LCL)were the constant display structures,and shown at a rate of 100%.ALL injury was correlated with the injury of ITB,Kaplan fiber bundle,lateral patellar retinaculum,PT,BT,PFL and LCL(P<0.05),but not with AL injury(P>0.05).Conclusion:The ALL is involved in a variety of sports of the knee joint,not only a secondary stabilizer limiting the internal rotation of the knee joint,but also participates in the external rotation of the knee joint,the forward movement of the tibia and the resistance to varus stress of the knee joint.
