摘要:
目的:探讨循环式单面光疗与间断式单面光疗治疗遗传性葡萄糖-6-磷酸脱氢酶(G6PD)缺乏所致新生儿高胆红素血症的临床疗效。方法:选取G6PD缺乏症所致高胆红素血症的新生儿150例,按照随机双盲法分为循环组和间断组。循环组给予循环式单面蓝光治疗,间断组给予间断式单面蓝光治疗。对比两组患儿临床疗效。结果:间断组患儿治疗有效率为96.00%,循环组患儿治疗有效率为93.33%,两组比较差异无统计学意义(P>0.05)。两组患儿经治疗后血清胆红素(STB)水平不断降低,组内0 h、24 h、72 h对比,差异均具有统计学意义(均P<0.05); 组间24 h、72 h对比,循环组STB水平均低于间断组(P<0.05)。治疗48 h后,循环组患儿Hb水平和红细胞计数均低于对照组(P<0.05)。循环组患儿不良反应发生率为6.67%,低于间断组的28.00%,差异具有统计学意义(P<0.05)。循环组患儿黄疸消退时间和住院时间均短于间断组(P<0.05)。结论:循环式单面光疗与间断式单面光疗对G6PD缺乏症所致高胆红素血症均具有较好的临床疗效,但前者有更多优势。
Abstract:
Objective:To investigate the clinical efficacy of cyclic single-sided phototherapy and intermittent single-sided phototherapy in treatment of neonatal hyperbilirubinemia caused by genetic G6PD deficiency.Methods:150 neonates with hyperbilirubinemia caused by G6PD deficiency were randomly divided into cyclic group and discontinuous group.The former was treated with cyclic single-sided phototherapy, and the latter was treated with intermittent single-sided phototherapy.The clinical efficacy were compared.Results:The effective rate was 96.00% in discontinuous group and 93.33% in cyclic group(P>0.05).The STB levels in two groups were continuously decreased after treatment, and the differences were statistically significant in 0, 24 and 72 hours(all P<0.05).The STB levels in cyclic group in 24 and 72 hours were lower than those in discontinuous group(P<0.05).After 48 hours of treatment, Hb level and red blood cell count in cyclic group were lower than those in discontinuous group(P<0.05).The incidence of adverse reactions in cyclic group was lower than that in discontinuous group(6.67% vs.28.00%)(P<0.05).The jaundice regression time and hospitalization time in cyclic group were shorter than those in discontinuous group(P<0.05).Conclusion:Cyclic single-sided phototherapy and intermittent single-sided phototherapy have good clinical effects on hyperbilirubinemia caused by G6PD deficiency, and can reduce the level of STB in children.However, the former has more advantages.
