摘要:
目的:探讨舒芬太尼静脉自控镇痛对腹腔镜肾癌根治术治疗晚期肾癌患者镇痛效果及对炎症反应的影响。方法:选择行腹腔镜肾癌根治术治疗的晚期肾癌术后患者102例,随机分为对照组和研究组,各51例; 对照组给予地佐辛镇痛,研究组给予舒芬太尼联合地佐辛镇痛; 比较两组干预前后视觉模拟评分法(VAS)、疼痛分级法(NRS)评分,检测两组干预前后血清中白介素-6(IL-6)、白介素-10(IL-10)、C反应蛋白(CRP)浓度及外周血淋巴细胞(LY)、白细胞(WBC)、血沉(ESR)水平,记录两组干预后不良反应发生情况。结果:干预后12、24 h研究组VAS、NRS评分均明显低于对照组(P<0.05),干预后血清中IL-6、IL-10、CRP水平明显低于对照组(P<0.05),外周血LY、WBC、ESR水平明显低于对照组(P<0.05),两组干预后不良反应发生率比较无统计学差异(P>0.05)。结论:舒芬太尼静脉自控镇痛能提高经腹腔镜肾癌根治术后患者的镇痛效果,降低炎症反应,且安全性较高。
Abstract:
Objective:To explore the analgesic effect of sufentanil patient-controlled intravenous analgesia(PCIA)on patients with advanced renal cancer undergoing laparoscopic radical nephrectomy and its influence on inflammatory response.Methods:A total of 102 patients with advanced renal cancer who underwent laparoscopic radical nephrectomy were randomly divided into control group and study group,with 51 cases in each group.The control group was given dezocine for analgesia,while the study group was given sufentanil combined with dezocine for analgesia.The scores of visual analogue scale(VAS)and numerical rating scale(NRS)before and after intervention were recorded and compared between the two groups.The concentration of serum interleukin-6(IL-6),interleukin-10(IL-10)and C-reactive protein(CRP),and the levels of lymphocytes(LY),white blood cell(WBC)and erythrocyte sedimentation rate(ESR)in peripheral blood before and after intervention in both groups were detected.Adverse reactions after intervention in both groups were recorded.Results:At 12 and 24 hours after intervention,VAS and NRS scores of study group were significantly lower than those of control group(P<0.05).After intervention,the levels of serum IL-6,IL-10 and CRP in study group were significantly lower than those in control group(P<0.05),and the levels of LY,WBC and ESR in peripheral blood were significantly lower than those in control group(P<0.05).After intervention,there was no significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion:Sufentanil PCIA can improve analgesic effect,and reduce inflammatory response in patients after laparoscopic radical nephrectomy,with relatively higher treatment safety.
