摘要:
目的:探究贝伐单抗联合小梁切除术与贝伐单抗联合全视网膜光凝术(PRP)治疗新生血管性青光眼(NVG)的临床疗效。方法:NVG患者94例,采用简单抽样法将其分为对照组和观察组,各47例。观察组球内注射贝伐单抗后给予小梁切除术治疗,对照组于注射贝伐单抗后给予PRP术治疗。比较两组治疗总有效率; 比较两组术前及术后15 d、1个月眼压、神经纤维层厚度、视野缺损值; 统计患者视力恢复情况及术后角膜水肿、前房积血等并发症发生情况; 比较两组手术前后视网膜静脉循环时间及虹膜新生血管消退时间。结果:观察组治疗总有效率、视力提高率高于对照组,并发症发生率低于对照组(P<0.05); 观察组术后15 d、1个月的眼压均低于对照组,神经纤维层厚度大于对照组,视野缺损值小于对照组(P<0.05); 观察组术后1个月的视网膜静脉循环时间短于对照组,观察组虹膜新生血管消退时间短于对照组(P<0.05)。结论:应用贝伐单抗联合小梁切除术治疗NVG有效率高,可提高患者视力、降低眼压、改善视网膜功能、降低并发症发生率。
Abstract:
Objective:To explore the clinical curative effect of bevacizumab combined with trabeculectomy and bevacizumab combined with panretinal photocoagulation(PRP)in the treatment of neovascular glaucoma(NVG).Methods:94 NVG patients were divided into control group and observation group by simple sampling method,47 cases in each group.The observation group was treated with trabeculectomy after eyeball injection of bevacizumab,while the control group was treated with PRP after bevacizumab injection.The total effective rate of treatment,intraocular pressure,nerve fiber layer thickness and visual field defect values before surgery,at 15 days and 1 month after surgery were compared between the two groups.The visual acuity recovery and postoperative complciations such as corneal edema and hyphema were statistically analyzed.The circulation time of retinal veins and fade time of iris neovascularization before and after surgery were compared between the two groups.Results:The total effective rate of treatment and improvement rate of visual acuity in the observation group were higher than those in the control group,while the incidence of complications was lower than that in the control group(P<0.05).At 15 days and 1 month after surgery,the intraocular pressure in the observation group was lower than that in the control group,and the nerve fiber layer thickness was greater than that in the control group,and visual field defect value was less than that in the control group(P<0.05).At 1 month after surgery,the circulation time of retinal veins in the observation group was shorter than that in the control group,and the fade time of iris neovascularization was shorter than that in the control group(P<0.05).Conclusion:The curative effect of bevacizumab combined with trabeculectomy is higher in the treatment of NVG,which can improve visual acuity,reduce intraocular pressure,improve retinal function,and reduce the incidence of complications.
