摘要:
目的:探讨宫颈高危型人乳头瘤病毒(HR-HPV)感染与阴道菌群及血清氧化应激水平的关系。方法:选取有生殖道感染症状的患者112例作为研究对象,采集宫颈分泌物检测HR-HPV感染情况、CD4+T与CD8+T淋巴细胞比例,检测宫颈阴道中病原菌分布情况,抽取外周血检测血清氧化应激水平及炎性因子浓度,分析HR-HPV感染与阴道菌群、局部免疫微环境及血清氧化应激状态的关系。结果:经宫颈分泌物检查结果显示,112例患者HR-HPV感染阳性患者47例(占41.96%); HR-HPV阳性组衣原体、解脲脲原体与细菌性阴道炎检出率明显高于HR-HPV阴性组(P<0.05); HR-HPV阳性组宫颈分泌物CD4+T淋巴细胞比例与CD4+T/CD8+T比值明显低于HR-HPV阴性组(P<0.05)。HR-HPV阳性组血清总氧化态(TOS)、氧化应激指数(OSI)、γ-干扰素(IFN-γ)与白介素2(IL-2)浓度明显高于HR-HPV阴性组(P<0.05),而HR-HPV阳性组血清总还原态(TAS)与白介素4(IL-4)浓度明显低于HR-HPV阴性组(P<0.05)。多因素Logisic回归分析结果显示,HR-HPV感染与衣原体、解脲脲原体感染率呈现独立正相关关系(P<0.05),与血清TOS、OSI、IFN-γ及IL-2浓度呈现独立正相关关系(P<0.05),与宫颈分泌物中CD4+T/CD8+T比值及血清TAS呈现负相关关系(P<0.05)。结论:宫颈HR-HPV感染患者阴道衣原体与解脲脲原体感染率升高,感染部位CD4+T/CD8+T比值降低,血清呈现高炎性因子水平与氧化应激状态。
Abstract:
Objective:To investigate the relationship between cervical high-risk human papillomavirus(HR-HPV)infection and vaginal flora and serum oxidative stress level.Methods:112 patients with reproductive tract infection were selected as the study objects.Cervical secretion was collected to detect the infection of HR-HPV,the proportion of CD4+T and CD8+T lymphocytes,the distribution of pathogenic bacteria in cervix and vagina.The level of serum oxidative stress and the concentration of inflammatory factors were detected in peripheral blood.The relationship between HR-HPV infection and vaginal flora,local immune microenvironment and serum oxidative stress was analyzed.Results:The results of cervical secretion examination showed that 47 of 112 patients were positive for HR-HPV infection,accounting for 41.96%; the detection rate of Chlamydia,Ureaplasma urealyticum and bacterial vaginitis in HR-HPV positive group was significantly higher than that in HR-HPV negative group(P<0.05); the ratio of CD4+T lymphocyte and CD4+T/CD8+T in HR-HPV positive group was significantly lower than that in HR-HPV negative group(P<0.05).The concentrations of total oxidation state(TOS),oxidative stress index(OSI),interferon gamma(IFN-γ)and interleukin-2(IL-2)in HR-HPV positive group were significantly higher than those in HR-HPV negative group(P<0.05),while the concentrations of total reduction state(TAS)and interleukin-4(IL-4)were significantly lower than those in the HR-HPV negative group(P<0.05).Multivariate Logistic regression analysis showed that HR-HPV infection was positively correlated with Chlamydia and Ureaplasma urealyticum infection rate(P<0.05),and positively correlated with the concentrations of serum TOS,OSI,IFN-γ and IL-2(P<0.05),and negatively correlated with CD4+T/CD8+T ratio and serum TAS in cervical secretion(P<0.05).Conclusion:The infection rate of Chlamydia and Ureaplasma urealyticum in the vagina of patients with cervical HR-HPV infection is increased,the ratio of CD4+T/CD8+T in the infected site is decreased,the high levels of inflammatory factors and oxidative stress state in serum are closely related to HR-HPV infection.
