摘要:
目的:应用二维斑点追踪技术(2D-STI)评价子痫前期(PE)对患者左心室机械形变及其同步性的影响。方法:收集69例PE患者,其中轻度PE患者38例作为MPE组,重度PE患者31例作为SPE组,并选择与其年龄、孕周等相匹配的正常健康孕妇40例作为对照组(CON组)。所有受试者首先应用超声心动图,并采用2D-STI检测左室各切面及整体水平收缩期长轴应变峰值(GLS),用应变达峰时间标准差表示机械离散度(MD)。结果:与CON组相比,MPE组和SPE组心尖四腔、三腔及两腔切面及整体GLS均降低,MD均升高; 而与MPE组相比,SPE组GLS明显降低及MD明显升高。结论:PE可导致患者左室收缩期机械运动的不同步性增加,形变程度降低,GLS及MD可作为检测PE患者心肌损伤的敏感指标。
Abstract:
Objective:To evaluate the effect of left ventricular mechanical deformation and synchrony in preeclampsia(PE)patients by two-dimensional speckle tracking(2D-STI).Methods:69 PE patients were collected,38 mild PE patients as MPE group,31 severe PE patients as SPE group,and 40 normal pregnant women matched with their age and gestational age as control(CON)group.All subjects first were examined by echocardiography,and 2D-STI was used to detect the peak value of global longitudinal strain(GLS)in left ventricular sections and the whole horizontal systolic period,and the standard deviation of strain peak time was used to express the mechanical dispersion(MD).Results:Compared with the CON group,the GLS in the four chamber,three chamber and two chamber sections and the whole GLS decreased in the SPE group and MPE group,and the MD increased.Compared with the MPE group,the GLS decreased and the MD increased significantly in the SPE group.Conclusion:PE can increase asynchrony of left ventricular systolic mechanical movement and reduced degree of deformation.GLS and MD can be used as sensitive indicators to detect myocardial injury in PE patients.
