摘要:
目的:探讨2型糖尿病患者血清乙酰肝素酶(HPA)、α-L-岩藻糖苷酶(AFU)和胱抑素C(Cys-C)水平预测糖尿病早期肾损害的临床价值。方法:将120例2型糖尿病患者根据尿白蛋白排泄率(UAER)平均分为三组:单纯糖尿病组(SDM),24 h尿蛋白排泄率<30 mg; 早期糖尿病肾病组(EDKD),24 h尿蛋白排泄率30~300 mg; 临床糖尿病肾病组(CDKD),24 h尿蛋白排泄率>300 mg。对三组患者进行血清HPA、血清AFU和血清Cys-C测定,并研究其与尿微量白蛋白的相关性。结果:早期糖尿病肾病组与单纯糖尿病组及临床糖尿病肾病组比较,血清AFU、HPA水平显著升高(P<0.05),而血清Cys-C水平无明显升高。临床糖尿病肾病组与单纯糖尿病组及早期糖尿病肾病组比较,血清Cys-C 、HPA水平显著升高(P<0.05),而血清AFU水平无明显升高。尿白蛋白与AFU呈负相关(P<0.001),与HPA、Cys-C呈正相关(P<0.001)。受试者工作特征(ROC)曲线分析表明,检测血清AFU、HPA、Cys-C预测糖尿病早期肾损害曲线下面积分别为0.728、0.813和0.624,预测敏感度分别为60%、62.5%和27.5%,预测特异度分别为77.5%、95%和88%。结论:2型糖尿病患者出现糖尿病肾损害时血清中AFU、HPA及Cys-C均出现不同程度的升高; 在预测糖尿病早期肾损害时,联合检测血清AFU及血清HPA的临床价值要明显高于单一检测血清Cys-C的临床价值。
Abstract:
Objective:To investigate the clinical value of serum heparinase(HPA),alpha-L-fucosidase(AFU)and cystatin C(Cys-C)in predicting early renal damage in patients with type 2 diabetes mellitus.Methods:According to the urinary albumin excretion rate(UAER),120 patients with type 2 diabetes mellitus were divided into simple diabetes group(SDM group,UAER less than 30 mg/24h),early diabetic nephropathy group(EDKD group,UAER between 30 mg/24h and 300 mg/24h)and clinical diabetic nephropathy group(CDKD group,UAER more than 30 mg/24h).Serum HPA,AFU and Cys-C were measured in three groups,and the correlation between them and urinary microalbumin was studied.Results:The serum AFU and HPA levels in the EDKD group were significantly higher than those in the SD group and the CDKD group(P<0.05),but the serum Cys-C level was not significantly higher.The serum levels of Cys-C and HPA in the CDKD group were significantly higher than those in the SD group and the EDKD group(P<0.05),while the level of serum AFU was not significantly higher.Urinary albumin was negatively correlated with AFU(P<0.001)and positively correlated with HPA and Cys-C(P<0.001).The ROC curve analysis showed that the area under the curve of serum AFU,HPA,and Cys-C were 0.728,0.813 and 0.624 respectively,and the predictive sensitivity was 60%,62.5% and 27.5% respectively,and the predictive specificity was 77.5%,95% and 88% respectively.Conclusion:The levels of serum AFU,HPA and Cys-C are increased in different degrees in type 2 diabetes mellitus patients with diabetic renal damage.The clinical value of combined detection of serum AFU and HPA is significantly higher than that of single detection of serum Cys-C in the prediction of early diabetic renal damage.
