摘要:
目的:探讨经椎间孔入路椎间融合(TLIF)术治疗退变性腰椎滑脱症(DLS)疗效及对患者OD指数、JOA评分的影响。方法:60例DLS患者,随机平均分为两组,对照组30例患者采用传统后路椎间融合术治疗,试验组30例患者采用TLIF术治疗,对比两组治疗优良率、血清肌酸激酶(CK)、C反应蛋白(CRP)指标、OD指数、JOA评分、并发症发生率。结果:试验组治疗优良率(93.33%)明显比对照组(60.00%)高,试验组术后3 d血清CK、CRP指标均明显比对照组低,试验组术后1个月OD指数明显比对照组低,JOA评分明显比对照组高,两组比较差异均有统计学意义(P<0.05); 试验组并发症发生率(10.00%)与对照组(6.67%)比较,差异无统计学意义(P>0.05)。结论:TLIF术可有效改善DLS患者腰椎功能,提高腰椎稳定性,减轻应激反应,且并发症较少。
Abstract:
Objective:To explore the therapeutic effect of transforaminal lumbar interbody fusion(TLIF)in the treatment of degenerative lumbar spondylolisthesis(DLS)and its effect on OD index and JOA score. Methods:60 DLS patients were randomly divided into experimental group and control group on average. The 30 patients in the control group were treated with traditional posterior interbody fusion. The 30 patients in the experimental group were treated with TLIF treatment. The excellent rate of treatment,serum CK,CRP index,OD index,JOA score,and incidence of complications were compared between the two groups. Results:The excellent rate of treatment in the experimental group(93.33%)was significantly higher than that in the control group(60.00%). Three days after operation,the serum CK and CRP indexes of the experimental group were significantly lower than those of the control group. One month after operation,the OD index of the experimental group was significantly lower than that of the control group,and the JOA score was significantly higher than that of the control group,the difference was statistically significant(P<0.05). There was no statistical difference in incidence of complications between the experimental group(10.00%)and the control group(6.67%)(P>0.05). Conclusion:TLIF can effectively improve lumbar function,improve lumbar spine stability,reduce stress response in patients with DLS,and has fewer complications.
