摘要:
目的:探讨吻合器痔上黏膜环切术(PPH)联合缚扎术治疗重度环状混合痔的临床效果及安全性。方法:选取重度环状混合痔患者76例,按随机数字表法分为观察组和对照组各38例,观察组采用PPH联合缚扎术治疗,对照组单用缚扎术治疗,统计两组患者临床疗效,并对两组手术时间、术中出血量、住院时间、术后症状缓解情况、术后并发症情况及复发情况进行分析。结果:观察组有效率为94.7%,高于对照组的71.0%,组间比较差异有统计学意义(P<0.05); 观察组的术中出血量、住院时间等指标均短于对照组,术后症状缓解率高于对照组,两组比较差异均有统计学意义(P<0.05); 观察组手术时间、术后尿潴留等指标与对照组比较差异无统计学意义(P>0.05); 观察组术后复发率低于对照组,组间比较差异有统计学意义(P<0.05)。结论:PPH联合缚扎术是治疗重度环状混合痔安全有效的方法。
Abstract:
Objective:To explore the therapeutic effect and safety of procedure for prolapse and hemorrhoids(PPH)combined with ligation in the treatment of severe circular mixed hemorrhoids. Methods:76 patients with severe circular mixed hemorrhoids were divided into observation group and control group,with 38 cases in each group. PPH combined ligation was used in the observation group,and ligation alone was used in the control group. The therapeutic effect of the two groups were statistically analyzed,and the operation time,transoperative bleeding volume,length of stay,postoperative symptom relief,postoperative complications and recurrence were analyzed.Results:The effective rate of the observation group(94.7%)was higher than that of the control group(71.0%)(P<0.05). The transoperative bleeding volume and length of stay in the observation group were shorter than those in the control group(P<0.05),and the postoperative symptom relief rate was higher than that in the control group(P<0.05),but there was no significant difference in the operative time and postoperative urinary retention between the two groups(P>0.05). The recurrence rate in the observation group was lower than that in the control group,with significant difference(P<0.05).Conclusion:PPH combined with ligation is a safe and effective treatment for severe circular mixed hemorrhoids.
