摘要:
目的:探讨子宫内膜异位症(EM)合并不孕患者血清中基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶抑制剂-1(TIMP-1)、血管内皮生长因子可溶性受体(sflt-1)、胰岛素生长因子-1(IGF-1)、单核细胞趋化蛋白-1(MCP-1)表达水平及临床意义。方法:收集50例单纯EM患者(单纯EM组)、35例EM合并不孕患者(EM合并不孕组)和30例健康体检(对照组)的育龄期女性作为受试对象。测定对比各组受试者血清MMP-9、TIMP-1、sflt-1、IGF-1、MCP-1水平及MMP-9/TIMP-1值,Pearson分析法分析各指标与EM合并不孕患者r-AFS分期相关性。结果:三组受试者排卵期血清MMP-9、TIMP-1、sflt-1、IGF-1、MCP-1水平及MMP-9/TIMP-1值间比较差异均具有统计学意义(P<0.05)。组间比较,单纯EM组MMP-9、IGF-1、MCP-1水平及MMP-9/TIMP-1值明显高于对照组,TIMP-1、sflt-1水平明显低于对照组(P<0.05); EM合并不孕组MMP-9、IGF-1、MCP-1水平及MMP-9/TIMP-1值明显高于单纯EM组和对照组,TIMP-1、sflt-1水平明显低于单纯EM组和对照组(P<0.05)。血清MMP-9、IGF-1、MCP-1水平及MMP-9/TIMP-1值与EM合并不孕患者r-AFS分期呈显著正相关(P<0.01),TIMP-1、sflt-1水平与r-AFS分期呈显著负相关(P<0.01)。结论:女性排卵期血清MMP-9、TIMP-1、sflt-1、IGF-1、MCP-1水平及MMP-9/TIMP-1值变化与EM及EM所致不孕发病有密切关系,临床可结合排卵期血清MMP-9、TIMP-1、sflt-1、IGF-1、MCP-1表达水平测定,更好的诊断EM合并不孕病情及评价预后。
Abstract:
Objective:To investigate the expression of matrix metalloproteinase-9(MMP-9),matrix metalloproteinase inhibitor-1(TIMP-1),vascular endothelial growth factor soluble receptor-1(sflt-1),insulin growth factor-I(IGF-1)and monocyte chemoattractant protein-1(MCP-1)in serum of endometriosis(EM)patients with infertility and its clinical significance. Methods:50 patients with simple EM(simple EM group),35 EM patients with infertility(EM with infertility group)and 30 healthy women(control group)were collected as subjects. The levels of serum MMP-9,TIMP-1,sflt-1,IGF-1,MCP-1 and the values of MMP-9/TIMP-1 were measured and compared between three groups. Pearson analysis was used to analyze the correlation between each index and r-AFS staging of EM patients with infertility. Results:There were significant differences in the levels of serum MMP-9,TIMP-1,sflt-1,IGF-1,MCP-1 and the values of MMP-9/TIMP-1 between the three groups(P<0.05). The levels of serum MMP-9,IGF-1,MCP-1 and the value of MMP-9/TIMP-1 in simple EM group were significantly higher than those in control group,but the levels of serum TIMP-1 and sflt-1 were significantly lower than those in control group(P<0.05). The levels of serum MMP-9,IGF-1,MCP-1 and the value of MMP-9/TIMP-1 in EM with infertility group were significantly higher than those in simple EM group and control group,but the levels of serum TIMP-1 and sflt-1 were significantly lower than those in simple EM group and control group(P<0.05). The levels of serum MMP-9,IGF-1,MCP-1 and the value of MMP-9/TIMP-1 were significantly positively correlated with r-AFS staging of EM patients with infertility(P<0.01),but the levels of TIMP-1,sflt-1 were significantly negatively correlated with r-AFS staging(P<0.01). Conclusion:The levels of serum MMP-9,TIMP-1,sflt-1,IGF-1,MCP-1 and the value of MMP-9/TIMP-1 in female ovulation stage are closely related to the incidence of infertility caused by EM and EM. The detection of MMP-9,TIMP-1,sflt-1,IGF-1 and MCP-1 in ovulation serum can be combined with the clinical measurement to better diagnose the condition of EM with infertility and evaluate the prognosis.
