摘要:
目的:探讨冠心病颈动脉超声参数与斑块稳定性及炎症反应水平间的相关性。方法:选择105例冠心病患者及30例健康体检者分别作为研究对象。同时依据冠心病患者Gensini评分将其分为轻度组、中度组和重度组3个亚组。采用彩色多普勒超声诊断系统测定各组颈动脉超声参数脉搏波传导速度(PWVβ)、颈动脉顺应性(CAC)、总动脉硬化程度(β)、颈动脉中层厚度(IMT),同时测定各组血清中斑块稳定性相关指标蛋白酶K(CatK)、I型胶原羧基末端肽(ICTP)、基质金属蛋白酶-2(MMP-2)、MMP-9及炎症反应指标超敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)水平,并采用Pearson相关性法分析颈动脉超声参数与斑块稳定性相关指标及炎症反应指标间相关性。结果:4组PWVβ、CAC、β、IMT、CatK、ICTP、MMP-2、MMP-9、hs-CRP、TNF-α、IL-6水平间差异具有统计学意义(P<0.05)。随着冠状动脉病变程度加重,受试者PWVβ、β、IMT、CatK、ICTP、MMP-2、MMP-9、hs-CRP、TNF-α、IL-6升高,即重度组>中度组>轻度组>对照组(P均<0.05); 而CAC值则逐渐下减小,即重度组<中度组<轻度组<对照组(P均<0.05)。Pearson相关性分析显示,冠心病颈动脉超声参数PWVβ、CAC、β、IMT与CatK、ICTP、MMP-2、MMP-9、hs-CRP、TNF-α、IL-6表达水平间均呈明显正相关,CAC则与其存在明显负相关(P均<0.05)。结论:颈动脉超声参数与冠心病患者冠状动脉粥样硬化斑块稳定性及炎症反应水平存在明显相关性,颈动脉超声检查可作为冠心病临床诊断及病情进展评估的可靠手段。
Abstract:
Objective:To explore the correlation between carotid ultrasound parameters and plaque stability and inflammatory response level in patients with coronary heart disease. Methods:105 patients with coronary heart disease were divided into mild group,moderate group and severe group according to the Gensini score. At the same time,30 healthy subjects for medical examination were selected as control group.The color Doppler ultrasound system was used to measure the pulse wave velocity(PWVβ),carotid artery compliance(CAC),total arteriosclerosis(β)and intima-media thickness(IMT)of the carotid artery. Meanwhile,the levels of cathepsin K(CatK),type Ⅰ collagen carboxy terminal telopeptide(ICTP),matrix metalloproteinase 2(MMP-2),MMP-9,hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor alpha(TNF-α),interleukin-6(IL-6)were determined. Pearson correlation method was used to analyze the correlation between carotid ultrasound parameters and plaque stability related indicators and inflammatory response indicators. Results:The differences of PWVβ,CAC,β,IMT,CatK,ICTP,MMP-2,MMP-9,hs-CRP,TNF-α and IL-6 levels in the four groups were statistically significant(P<0.05). PWVβ,β,IMT,Catk,ICTP,MMP-2,MMP-9,hs-CRP,TNF-α,IL-6 increased with the severity of coronary artery disease,that is,severe group > moderate group > mild group > control group(P<0.05). However,CAC value gradually decreased,that is,severe group< moderate group< mild group< control group(both P<0.05). Pearson correlation analysis showed that PWVβ,β,IMT were positively correlated with CatK,ICTP,MMP-2,MMP-9,hs-CRP,TNF-α and IL-6,while CAC was negatively correlated with them(all P<0.05). Conclusion:Carotid ultrasound parameters have a significant correlation with coronary atherosclerotic plaque stability and inflammatory response in patients with coronary heart disease. Carotid ultrasound can be used as a reliable means for clinical diagnosis and assessment of the progress of coronary heart disease.
