摘要:
的:探究老年动脉硬化性心肌梗死(MI)患者血清胱抑素C(Cys C)水平与血管内皮细胞损伤的相关性。方法:以收入的42例MI患者(MI组)、35例心绞痛患者(心绞痛组)和同期体检的35例健康志愿者(对照组)为研究对象。比较各组基础生化指标水平; 测量各组受试者Cys C和血管内皮细胞损伤指标一氧化氮(NO)、内皮素-1(ET-1)、纤溶酶原激活物抑制剂-1(PAI-1)、血栓调节蛋白(TM)水平,分析Cys C表达与基础生化指标及血管内皮细胞损伤指标相关性。结果:三组血清肌酸激酶(CK)、总胆固醇(CHOL)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、Cys C、NO、ET-1、API-1、TM水平间比较差异具有统计学意义(P<0.01)。MI组血清CK水平明显高于心绞痛组和对照组(P<0.01),但心绞痛组和对照组间差异无统计学意义(P<0.05); MI组和心绞痛组血清CHOL、TG、LDL-C、Cys C、ET-1、API-1、TM水平均显著高于对照组,NO水平低于对照组(P<0.01); MI组和心绞痛组血清CHOL、TG、LDL-C水平间比较差异无统计学意义(P>0.05),但Cys C、ET-1、API-1、TM水平均高于心绞痛组,NO水平低于心绞痛组(P<0.01)。Pearson相关系数分析显示,MI患者血清Cys C水平与CK、CHOL、TG、LDL-C、ET-1、PAI-1和TM均呈显著正相关,与NO水平呈显著负相关(P<0.01)。结论:老年动脉硬化性MI患者血清Cys C水平与血管内皮损伤标志物ET-1、PAI-1和TM呈显著正相关,与NO呈显著负相关。
Abstract:
Objective:To investigate the correlation between serum cystatin C(Cys C)level and vascular endothelial cell injury in elderly patients with atherosclerotic myocardial infarction(MI). Methods:42 patients with MI(MI group),35 patients with angina pectoris(angina pectoris group)and 35 healthy volunteers(control group)for physical examination in the same period were included in the study. The levels of basic biochemical indicators in each group were compared. The level of Cys C,nitric oxide(NO),endothelin-1(ET-1),plasminogen activator inhibitor-1(PAI-1)and thrombomodulin(TM)were measured. The correlation between Cys C expression and basic biochemical indicators and vascular endothelial cell injury indexes was analyzed. Results:The differences of serum CK,CHOL,TG,LDL-C,Cys C,NO,ET-1,API-1 and TM levels in the three groups were statistically significant(P<0.01).The serum CK level in MI group was significantly higher than that in angina pectoris group and control group(P<0.01),but there was no significant difference between angina pectoris group and control group(P<0.05). The levels of serum CHOL,TG,LDL-C,Cys C,ET-1,API-1 and TM in MI group and angina pectoris group were significantly higher than those in control group,and the NO level was lower than that in control group(P<0.01). There was no significant difference in the levels of serum CHOL,TG and LDL-C between MI group and angina pectoris group(P>0.05),but the Cys C,ET-1,API-1,TM levels were higher than those in angina group,and the NO level was lower than that in angina group(P<0.01). Pearson correlation analysis showed that serum Cys C level were positively correlated with CK,CHOL,TG,LDL-C,ET-1,PAI-1 and TM significantly,and negatively correlated with NO level(P<0.01). Conclusion:Serum Cys C level in elderly patients with arteriosclerotic MI were positively correlated with vascular endothelial injury markers ET-1,PAI-1 and TM significantly,and negatively correlated with NO.
