摘要:
目的:初步探讨一组家庭聚集性2019新型冠状病毒肺炎(COVID-19)患者初次就诊临床检验与HRCT表现。方法:回顾性分析最终确诊的家庭聚集性发病的新冠肺炎患者8例(男2例,女6例)初次就诊时临床检验和HRCT表现,统计检验结果的变化,分析CT特异征象。结果:8例患者中白细胞计数正常或减低(8例),单核细胞比率升高(5/8例),淋巴细胞数及比率大多降低(5/8例),中性细胞数均正常,高敏C反应蛋白升高(6/8例); 血小板均正常,甲型乙型流感病毒抗原检测及肺炎支原体抗体检测均为阴性。CT征象除外1例其余均为早期及进展期,7例患者可见两肺外带斑片状磨玻璃密度影(GGO)伴增粗小血管影,其中2例出现磨玻璃样伴部分实变,8例患者均无胸腔积液。结论:家庭聚集性新冠肺炎的感染人群初次就诊时实验室及HRCT检查结果均具有一定特征,两者联合应用可以对具有临床症状、密切接触者大范围早期筛查,为进一步核酸检测提供依据。
Abstract:
Objective:To investigate the laboratory examination and HRCT features of family aggregative corona virus disease 2019(COVID-19)patients at first visit. Methods:The laboratory examination and HRCT features of 8 patients(2 males and 6 females)with family aggregative COVID-19 were analyzed retrospectively. Results:The white blood cell count was normal or decreased in 8 patients(8 cases),the ratio of monocytes was increased(5/8 cases),the number and ratio of lymphocytes were mostly decreased(5/8 cases),the number of neutrophils was normal,and the high-sensitivity C-reactive protein was increased(6/8 cases). Platelets were normal,and both the detection of influenza A and B virus antigens and the detection of Mycoplasma pneumoniae antibody were negative. Except for 1 case,CT signs were early and advanced,7 cases of patients had plaque-like ground-glass density(GGO)with thickened small blood vessels in two lung periphery,of which 2 cases had ground-glass-like appearance with partial consolidation. There were no pleural effusions in 8 patients. Conclusion:The results of laboratory and HRCT examinations at first visit of the infected population with family aggregative COVID-19 have certain characteristics. The combined application of the two can provide a wide range of early screening for patients with clinical symptoms and close contacts,and provide a basis for further nucleic acid detection.
