摘要:
目的:观察中老年2型糖尿病(T2DM)患者尿白蛋白/肌酐比值( UACR)对认知功能的影响,探讨二者的相关性。方法:选取中老年T2DM患者136例,根据神经认知功能测定,分为正常组66例,认知功能障碍组70例。对所有对象进行问卷调查及实验室检查,比较两组之间差异,并采用单因素分析和多因素Logistic回归分析筛选中老年T2DM患者认知功能障碍相关影响因素。结果:单因素分析:认知功能障碍组患者与正常组在空腹血糖(FBG)、糖化血红蛋白(HbA1c)、UACR、文化程度、合并抑郁状态等方面比较差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,T2DM患者认知功能障碍与高FBG、HbA1c和UACR、受教育年限低、高龄相关(P<0.05)。结论:T2DM患者UACR与认知功能损害关系密切,临床上通过对其检测可早期诊断及预防中老年T2DM合并认知功能损害。
Abstract:
Objective:To observe the effect of urine albumin-creatinine ratio(UACR)on cognitive function in middle-aged and to elderly patients with type 2 diabetes mellitus(T2DM),and to explore their relationship.Methods:136 patients with T2DM were selected and divided into normal function control group(66 cases)and cognitive dysfunction group(70 cases)according to the neurocognitive function.All subjects were investigated by questionnaires and laboratory tests,and the differences between the two groups were compared.Single-factor and multi-factor Logistic regression analysis were used to screen the related influencing factors of cognitive dysfunction in T2DM patients.Results:Single-factor regression analysis showed that serum fasting blood glucose(FBG),hemoglobin A1c(HbA1c),UACR,education level,depression symptoms in cognitive dysfunction group were statistically different from those in normal function control group(P<0.05).Multi-factor Logistic regression analysis showed that cognitive dysfunction in type 2 diabetic patients was associated with high levels of FBG,HbA1c,and UACR,as well as low years of education and advanced age(P<0.05).Conclusion:The UACR is closely related to the cognitive function of type 2 diabetes mellitus.The early diagnosis and prevention of patients with type 2 diabetes mellitus can be achieved through the detection of UACR.
