摘要:
目的:研究血管内皮生长因子(VEGF)、转化生长因子-β1(TGF-β1)、糖抗原19-9(CA19-9)、糖抗原242(CA242)和分泌型蛋白Dickkopf-1(DKK1)联合检查对早期胰腺癌患者的临床诊断价值。方法:选取胰腺癌患者60例作为病变组,胰腺炎患者60例作为对照组。比较两组血清VEGF、TGF-β1、CA19-9、CA242和DKK1水平,分析不同检查方式诊断胰腺癌的价值,并分析血清VEGF、TGF-β1、CA19-9、CA242、DKK1水平与胰腺癌患者病理特征的关系。结果:病变组及对照组在血清VEGF、TGF-β1、CA19-9、CA242和DKK1水平方面对比,前者高于后者(均P<0.05)。血清VEGF、TGF-β1、CA19-9、CA242和DKK1联合检测诊断胰腺癌的灵敏度、特异度、准确度分别为95.00%、91.67%、93.33%,相较于上述各项血清学指标单独检测明显提高(均P<0.05)。肿瘤分期为Ⅲ～Ⅳ期胰腺癌患者血清VEGF、TGF-β1、CA19-9、CA242、DKK1水平相较于Ⅰ～Ⅱ期患者更高(均P<0.05)。结论:血清VEGF、TGF-β1、CA19-9、CA242和DKK1联合检查应用于早期胰腺癌的诊断,具有较高的灵敏度、特异度、准确度,有利于临床判断胰腺癌患者病情的严重程度。
Abstract:
Objective:To study the clinical diagnostic value of combined examination of vascular endothelial growth factor(VEGF),transforming growth factor -beta 1(TGF-β1),carbohydrate antigen 19-9(CA19-9),carbohydrate antigen 242(CA242)and secreted protein dickkopf-1(DKK1)in patients with early pancreatic cancer. Methods:A total of 60 patients with pancreatic cancer were selected as the lesion group. Another 60 patients with pancreatitis admitted to our hospital at the same time were taken as the control group. Serum VEGF,TGF-β1,CA19-9,CA242 and DKK1 levels were compared between the two groups. The diagnostic value of different examination methods for pancreatic cancer was analyzed,and the correlation between serum VEGF,TGF-β1,CA19-9,CA242 and DKK1 levels and pathological characteristics of pancreatic cancer patients was done. Results:The serum levels of VEGF,TGF-β1,CA19-9,CA242 and DKK1 were higher in the lesion group than those in the control group(all P<0.05).The sensitivity,specificity and accuracy of the combined detection of serum VEGF,TGF-β1,CA19-9,CA242 and DKK1 in the diagnosis of pancreatic cancer were 95.00%,91.67% and 93.33%,respectively,which were significantly higher than the detection of the above serum blood indicators alone(all P<0.05). The levels of serum VEGF,TGF-β1,CA19-9,CA242 and DKK1 in patients with stage III-IV pancreatic cancer were higher than those in stage I-II(P<0.05).Conclusion:Combined examination of VEGF,TGF-β1,CA19-9,CA242 and DKK1 in the diagnosis of early pancreatic cancer has high sensitivity,specificity and accuracy,which is conducive to clinical judgment of the severity of pancreatic cancer patients.
