摘要:
目的:探讨手术治疗前后外周血中性粒细胞淋巴细胞比值(NLR)的动态变化对卵巢癌预后的预测价值。方法:回顾性分析107例上皮性卵巢癌患者的临床病理资料,所有患者均于术前及术后第1次化疗前进行血常规检查并计算NLR值,分析NLR动态变化与患者临床病理特征和预后的相关性。结果:手术治疗前后NLR动态变化仅与术后残留病灶大小(P=0.009)、化疗敏感性(P=0.027)及复发状态(P=0.043)相关,而与年龄、FIGO分期、病理分级、恶性腹水、血清CA125水平及转移状态无关(P>0.05)。Kaplan-Meier生存分析显示,术后NLR升高者的总生存(OS)及无复发生存(RFS)时间明显短于NLR降低者,差异有统计学意义(P<0.001)。多因素生存分析显示NLR动态变化是影响卵巢癌RFS的独立因素(HR:1.836,95% CI:1.086～3.105,P=0.023)。结论:术后NLR升高提示上皮性卵巢癌患者预后不良,动态监测其变化对卵巢癌患者的预后有重要的预测价值。
Abstract:
Objective:To investigate the predictive value of dynamic changes in peripheral blood neutrophil lymphocyte ratio(NLR)before and after surgical treatment for the prognosis of ovarian cancer. Methods:The clinical and pathological data of 107 patients with epithelial ovarian cancer were analyzed retrospectively. Blood routine tests were taken to obtain the NLR values before surgery and the first cycle of chemotherapy after surgery. Then the correlation between dynamic changes in NLR and clinicopathological parameters and prognosis of patients were further assessed. Results:The dynamic changes of NLR before and after surgery were only related to the size of residual lesions(P=0.009),chemotherapy sensitivity(P=0.027),and recurrence status(P=0.043),but not to other clinicopathologic factors,including age,FIGO stage,pathological grade,malignant ascites,serum CA125 level,and metastasis status(all P>0.05). Survival analysis by Kaplan-Meier method demonstrated that patients with increased NLR after operation presented worse recurrence free survival(RFS)and overall survival(OS)than those with decreased NLR(P<0.001). Multivariate analysis suggested that dynamic change of NLR remained an independent factor for RFS(HR:1.836; 95%CI:1.086 to 3.105; P=0.023). Conclusion:Elevated NLR indicates poor prognosis in patients with epithelial ovarian cancer. Dynamic monitoring of its changes has important predictive value for the prognosis of ovarian cancer patients.
