摘要:
目的:观察保温措施对全麻肾移植患者术中体温和苏醒的影响。方法:60例全身麻醉肾移植患者,随机分为两组。观察组患者在手术期间采用电子保温毯和输液加温器等保温措施,对照组无保温措施。记录两组患者在麻醉诱导前(T0)、移植肾血管开放前(T1)、移植肾血管开放后30 min(T2)、术毕(T3)、苏醒时(T4)和离室时(T5)的体温变化,并比较两组恢复自主通气时间、苏醒时间、拔管时间、离室时间和苏醒期躁动评分。结果:对照组组内比较:与T0相比较,T1相体温变化差异无统计学意义(P>0.05),剩余时间点体温与T0相比,体温明显有所降低(P<0.05); 观察组组内比较:与T0相比,T1相体温变化差异无统计学意义(P>0.05),剩余时间点体温与T0相比,体温明显有所回升(P<0.05)。与对照组比较,观察组自主通气恢复时间、苏醒时间、拔管时间和离室时间明显缩短(P<0.05),苏醒期躁动评分也明显降低(P<0.05)。结论:围术期对肾移植患者采用保温措施可明显降低低体温的发生,缩短患者麻醉恢复时间,促进患者术后麻醉后苏醒。
Abstract:
Objective:To observe the effect of insulation on the body temperature and recovery in patients with kidney transplantation under general anesthesia. Methods:60 patients with kidney transplantation under general anesthesia were randomly divided into two groups,with 30 cases in each group. Patients in the observation group adopted thermal insulation measures such as electronic insulation blankets and infusion warmers during the operation,while the control group had no insulation measures. The body temperature changes were recorded before anesthesia induction(T0),before vascular anastomosis opening(T1),30 minutes after vascular anastomsis opening(T2),after the surgery(T3),at wake-up time(T4)and at departure time from recovery room(T5).The spontaneous breathing recovery time,wake-up time,extubation time,departure time from recovery room,and agitation score in two groups were compared. Results:In the control group,compared with T0,the body temperature had no obviously changes at T1(P>0.05),but dramatically decreased at the other times(P<0.05). In the observation group,compared with T0,the body temperature had no obviously changes at T1(P>0.05),but increased at other times(P<0.05). Compared with the control group,the spontaneous breathing recovery time,wake-up time,extubation time,and departure time from recovery room were shorter in the observation group(P<0.05),and the agitation score was lower in the observation group(P<0.05). Conclusion:Perioperative use of insulation measures for renal transplant patients can significantly reduce the occurrence of hypothermia,shorten the recovery time of anesthesia,and promote patients to wake up after anesthesia.
