目的:探讨稳定性冠心病(CHD)患者血清抵抗素水平与心肌梗死的相关性,研究血清抵抗素水平对心肌梗死发病的预测价值。方法:回顾性分析90例稳定性CHD患者的临床资料,根据随访2年期间是否并发心肌梗死分为梗死组(n=37)、CHD组(n=53)。比较两组患者基本临床资料、入院时血糖(FBG)、血脂、同型半胱氨酸(HCY)、肌酸激酶同工酶(CK-MB)、抵抗素等实验室指标,利用多因素Logistic回归分析血清抵抗素与心肌梗死的相关性,利用ROC曲线评价血清抵抗素对心肌梗死的预测价值。结果:与CHD组比较,梗死组患者年龄、病程、冠脉病变支数、FBG、左心室射血分数(LVEF)、三酰甘油(TG)、HCY、抵抗素水平均明显升高(P<0.05); 多因素Logistic回归分析显示,冠脉病变支数、HCY、抵抗素均是稳定性CHD并发心肌梗死的独立危险因素(P<0.05)。联合变量预测稳定性CHD患者并发心肌梗死的AUC、灵敏度、特异度均显著高于HCY、抵抗素单独预测(P<0.05)。结论:稳定性CHD合并心肌梗死患者血清抵抗素水平明显升高,可作为并发心肌梗死的重要预测指标。
Abstract:
Objective:To investigate the correlation between level of serum resistin and myocardial infarction in patients with stable coronary heart disease(CHD),and to explore the predictive value of serum resistin level for the incidence of myocardial infarction.Methods:The clinical data of 90 patients with stable CHD treated were retrospectively analyzed.According to whether myocardial infarction was complicated during the 2-year follow-up period,all the patients were divided into the infarction group(n=37)and CHD group(n=53).Basic clinical data,blood glucose,blood lipids,HCY,CK-MB,resistin and other laboratory indexes of the two groups of patients were compared.The correlation between serum resistin and myocardial infarction was analyzed by multivariate Logistic regression,and the predictive value of serum resistin for myocardial infarction was evaluated by ROC curve.Results:Compared with the CHD group,age,course of disease,number of coronary lesions,FBG,LVEF,TG,HCY and resistin levels in the infarction group were significantly increased(P<0.05).Multivariate Logistic regression analysis showed that the number of coronary artery lesions,HCY and resistin were all independent risk factors for stable CHD complicated with myocardial infarction(P<0.05).The AUC,sensitivity and specificity of combined variables in predicting stability of CHD patients complicated with myocardial infarction were significantly higher than those of HCY and resistin alone(P<0.05).Conclusion:Patients with stable CHD complicated with myocardial infarction have significantly increased serum resistin levels,which can be used as an important predictor of complicated myocardial infarction.
