摘要:
目的:探讨关节镜与传统切开手术治疗胫骨平台骨折合并前交叉韧带撕脱的效果及对患者HSS评分的影响。方法:选取胫骨平台骨折合并前交叉韧带撕脱的患者74例,随机分为研究组与对照组。对照组采用传统的切开手术治疗,研究组采用关节镜下的复位内固定术治疗。比较两组患者术后的恢复情况、膝关节功能评分、临床效果以及两组患者术后并发症发生情况。结果:研究组患者的术中出血量、骨折愈合时间以及住院时间均少于对照组,而手术时间长于对照组(P<0.05); 研究组的优良率高于对照组(P<0.05); 研究组的HSS以及Rasmussen评分均高于对照组(P<0.05),两组患者的TPA与PA评分无统计学差异(P>0.05); 研究组并发症总发生率低于对照组(P<0.05)。结论:对胫骨平台骨折合并前交叉韧带撕脱的患者采用关节镜辅助的手术治疗,能够显著降低患者术中出血量,缩短骨折愈合时间及住院时间,改善患者膝关节功能。
Abstract:
Objective:To explore the efficacy of arthroscopy and conventional incision surgery in the treatment of tibial plateau fracture with anterior cruciate ligament avulsion and the effect on HSS score for patients.Methods:74 cases of tibial plateau fracture with anterior cruciate ligament avulsion were randomly divided into study group and control group.The control group was treated with conventional incision surgery,and the study group was treated with arthroscopic reduction and internal fixation.The postoperative recovery,knee function score,clinical effect and postoperative complications of the two groups were compared.Results:The intraoperative blood loss,fracture healing time and hospitalization time of the study group were lower than those of the control group,and the operation time was longer than that of the control group(P<0.05).The excellent and good rate of the study group was higher than that of the control group(P<0.05).The HSS and Rasmussen scores of the study group were higher than those of the control group(P<0.05).There was no significant difference in TPA and PA scores between the two groups(P>0.05).The total incidence rate of complications in the study group was lower than that in the control group(P<0.05).Conclusion:Arthroscopically assisted surgery for patients with tibial plateau fracture combined with anterior cruciate ligament avulsion can significantly reduce intraoperative blood loss,shorten fracture healing and hospitalization time,and improve knee function.
