摘 要 目的：探究血液透析及血液灌注对糖尿病肾病患者血清空腹血糖（FPG）、C反应蛋白（CRP）、白细胞介素6（IL6）水平的影响；方法：选择96例糖尿病肾病患者为研究对象，按照随机数字表法将其均分为试验组与对照组（每组各48例患者），对照组患者单纯接受血液灌注治疗，试验组患者在对照组基础上加用血液灌注治疗，对比两组患者治疗前后CRP、IL6、血红蛋白（Hb）、白蛋白（Alb）、FPG、空腹胰岛素（FINS）水平，对比两组患者治疗前后主观综合性营养评分（SGA）及体质量指数（BMI）。结果：①治疗前两组患者CRP、IL6水平对比差异不具有统计学意义（P＞005），治疗后试验组患者CRP及IL6水平均低于对照组（P＜005）；②治疗前两组患者Hb和Alb水平对比差异不具有统计学意义（P＞005），治疗后试验组患者Hb及Alb水平均高于对照组（P＜005）；③治疗前两组患者SGA评分及BMI指数对比差异不具有统计学意义（P＞005），治疗后试验组患者SGA评分及BMI指数均高于对照组（P＜005）；④治疗前两组患者FBG及FINS水平对比差异不具有统计学意义（P＞005），治疗后试验组患者FBG及FINS水平均低于对照组（P＜005）。结论：血液透析联合血液灌注能够显著缓解糖尿病肾病患者炎性状态，改善其营养状况及胰岛素抵抗现象，有临床意义。
Abstract:
Abstract Objective:To explore the effect of hemodialysis and blood perfusion on the levels of fasting plasma glucose(FPG),Creactive protein(CRP) and interleukin6(IL6) in patients with diabetic nephropathy. Methods:96 diabetic nephropathy patients who were treated in our hospital from May 2018 to may 2019 were selected as the research objects. According to the random number table method,they were divided into the experimental group and the control group(48 patients in each group). The patients in the control group were only treated with hemodialysis. The patients in the experimental group were treated with blood perfusion on the basis of the control group. The CRP,IL6,Hb,Alb,FPG,FINS,SGA and BMI of the patients in the two groups were compared before and after treatment. Results:①there were no significant differences in the levels of CRP and IL6 between the two groups before treatment(P＞005). After treatment,the levels of CRP and IL6 in the experimental group were lower than those in the control group(P＜005).②there were no significant differences in the levels of Hb and Alb between the two groups before treatment(P＞005). After treatment,the levels of Hb and Alb in the experimental group were higher than those in the control group(P＜005).③There were no significant differences in SGA score and BMI index between the two groups before treatment(P＞005). After treatment,the SGA score and BMI index in the experimental group were higher than those in the control group(P＜005).④There were no significant differences in the level s of FBG and FINS between the two groups before treatment(P＞005). After treatment,the FBG and FINS level in the experimental group were lower than those in the control group(P＜005). Conclusion:Hemodialysis combined with blood perfusion can significantly alleviate the inflammatory state of diabetic nephropathy patients,improve their nutritional status and insulin resistance,which is worthy of clinical promotion.
