摘要:
摘 要 目的：探讨超声引导神经阻滞联合全身麻醉在胫骨骨折患者手术中的麻醉效果及对应激反应的影响。方法：胫骨骨折患者150例，随机分为观察组（n=75）与对照组（n=75），观察组采用超声引导神经阻滞联合全身麻醉，对照组单纯全身麻醉，对比两组麻醉效果及应激反应指标。结果：观察组拔管后10 min的Ramsay镇静评分为（239±018）分，明显优于对照组的（192±023）分，拔管后30 min的疼痛（VAS）评分为（185±075）分，明显低于对照组的（408±069）分（P＜005）。观察组唤醒时间（790±213）min、拔管时间（1038±262）min均早于对照组的（1353±319）min、（1755±382）min（P＜005）。观察组与对照组T0时刻的血浆血糖（GLU）、白介素6（IL6）、皮质醇（COR）对比差异无统计学意义（P＞005）；观察组与对照组T1~T4时刻的GLU、IL6、COR均明显升高，但观察组GLU、IL6、COR水平均明显较对照组低（P＜005）。观察组术后并发症发生率（267%）明显低于对照组（1467%）（P＜005）。结论：相比于单纯全身麻醉，超声引导神经阻滞与全身麻醉联合应用的麻醉效果更加显著，且减轻手术应激反应。
Abstract:
Abstract Objective：To investigate the anesthetic effect of ultrasound guided nerve block combined with general anesthesia in patients with tibial fracture and its effect on stress responseMethods：150 cases of tibial fracture admitted were selected，they were randomly divided into observation group （n=75） and control group （n=75）.The observation group was performed by ultrasound guided nerve block combined with general anesthesia，and the control group was given general anesthesia.The anesthetic effect and the stress response index of the two groups were comparedResults：10 minutes after extubation，the Ramsay score in the observation group （239±018） was significantly better than that in the control group （192±023），and 30 minutes after extubation the VAS score in the observation group （185±075） was significantly lower than that in the control group （408±069） （P＜005）.The wakeup time in the observation group （790±213） min and the extubation time （1038±262） min were earlier than those in the control group （1353±319） min and （1755±382） min（P＜005）.At T0，plasma glucose （GLU），interleukin6 （IL6） and cortisol （COR） were compared between the observation group and the control group，and the difference was not statistically significant （P＞005）.At time T1~T4，GLU，IL6 and COR were significantly increased in the observation group and the control group，but they were significantly lower in the observation group than those in the control group （P＜005）.Postoperative complications （267%） in the observation group were significantly lower than those in the control group （1467%）（P＜005）Conclusion：Compared with general anesthesia，ultrasound guided nerve block combined with general anesthesia is more effective and relieves surgical stress response
