摘要:
摘 要 目的:探讨高危型人乳头瘤状病毒(HRHPV)感染与阴道微生态及宫颈局部细胞免疫的相关性。方法:HPV感染的251患者,根据是否感染HRHPV分为HRHPV组(n=114例)、非HRHPV组(n=137例),另选择健康体检者作为对照组(n=100)。比较各组阴道微生态菌群分布及宫颈局部组织T淋巴细胞亚群变化情况。结果:HRHPV阳性患者以单一型感染为主,其中HPV 16占3860%,HPV 18占2807%,HPV 33占1842%。HRHPV组阴道微生态失调率高于非HRHPV组、对照组,非HRHPV组显著高于对照组(P<005)。HRHPV组、非HRHPV组阴道乳酸杆菌异常率及各菌群阳性率均高于对照组,HRHPV组乳酸杆菌异常率、衣原体和解脲脲原体阳性率高于非HRHPV组(P<005)。与对照组比较,HRHPV组、非HRHPV组CD3+、CD4+、CD8+、CD4+/CD8+均明显降低,而HRHPV组各T淋巴细胞亚群指标明显低于非HRHPV组(P<005)。结论:HRHPV感染以HRHPV16、18、33型为主,可加重患者阴道微生态紊乱程度,降低宫颈局部细胞免疫功能。
Abstract:
Abstract Objective:To explore the correlation of highrisk HPV(HRHPV) infection with vaginal microecology and cervical local cellular immunity function Methods:251 patients with HPV infection were divided into the HRHPV group (n=114) and non HRHPV group (n=137) according to whether infected with HRHPV,and the healthy subjects were selected as control group (n=100) The distribution of vaginal microflora and the changes of T lymphocyte subsets in cervical tissues were compared and analyzed Results:HRHPV positive patients were mainly for single type including HPV 16 accounted for 3860%,HPV 18 for 2807%,and HPV 33 for 1842% The vaginal microecologic imbalance rate of the HRHPV group was higher than that of the non HRHPV group and control group,and non HRHPV group was significantly higher than that of the control group (P<005) The abnormal rate of lactobacillus vaginalis and positive rate of each flora in HRHPV group and non HRHPV group were higher than those in control group,and the abnormal rate of lactobacillus vaginalis,chlamydia and ureaplasma in HRHPV group were higher than those in non HRHPV group (P<005) Compared with control group,CD3+、CD4+、CD8+、CD4+/CD8+ were significantly reduced in HRHPV group and non HRHPV group,while the T lymphocyte subsets in HRHPV group were significantly lower than those in non HRHPV group (P<005) Conclusion:HRHPV infection is mainly caused by HRHPV 16,18 and 33 types,which can aggravate the degree of vaginalmicro ecological disorder and reduce the cervical local cellular immunity function
