摘要:
摘 要 目的：系统评价胸痛血管三联成像与选择性CT诊断准确性、图像质量、辐射剂量、显影剂剂量。方法：检索中英文数据库关于急性肺创伤性胸痛使用胸痛血管三联成像与选择性CT图像质量、诊断准确性、辐射暴露剂量、显影剂剂量的研究，检索时限截止2018年12月，纳入文献质量以Quality Assessment of Diagnostic Accuracy Studies进行评估，以Review Manager及MetaDiSc软件对纳入研究进行分析。结果：最终纳入14篇研究；Meta结果示，TROCT诊断急性冠脉综合征的灵敏度为0.943，95%CI(0.891,0.97.5);特异度为0.974，95%CI(0.961,0.984);阳性似然比为17.71，95%CI(3.92,79.96);阴性似然比为0.08，95%CI(0.02,0.27);缺乏足够关于急性主动脉夹层、肺动脉栓塞的诊断数据；使用TROCT，图像质量无差别（OR=0.78,95%CI:0.53,1.02）;多产生平均6.00mSv的辐射剂量（95%CI:3.17,8.83）；多注射显影剂27.58 ml（95%CI：26.85,28.30）。结论：胸痛血管三联成像对于胸痛的鉴别及诊断有较高的应用价值。
