摘 要 目的:针对榆林地区手足口病的流行病学和病因学进行分析,以便制定相应的干预措施。方法:收集2014-2018年榆林地区手足口病流行病学和临床资料。分析手足口病患者的年龄、性别、发病季节、病原血清型和病情严重程度的分布,以描述其流行病学特征。对所有重症病例和随机选择的普通手足口病病例,收集咽喉或直肠拭子或粪便,通过实时逆转录聚合酶链反应检测肠道病毒,以识别致病因子。结果:2014-2018年,榆林市共报告手足口病30842例,其中重症病例227例(0.7%)。年平均发病率为166.71/100 000。在所有病例中,男女比例约为1.50∶1,3岁以下儿童和5岁以下儿童分别占66.3%和91.5%。每年4~7月为发病高峰,2016-2018年每年11月观察到的病例数略有增加。实验室确诊1004例(3.26%),在所有肠道病毒中,主要血清型为EV71,占39.4%,其次是CA16 占36.6%,其它肠道病毒占24.0%。在重症病例中,EV71(84.1%)是手足口病的主要病原体。结论:手足口病在榆林地区仍然是一个重要的公共卫生问题。在2014-2018年的5年期间,榆林地区手足口病的发病率略有上升。由于约40%的确诊病例由EV71引起的,因此EV71疫苗接种可能对手足口病的防治具有积极作用。
