摘要:
摘 要 目的：观察类风湿关节炎合并心血管损伤的危险因素。方法：纳入252例类风湿关节炎患者为研究对象，开展回顾性分析。统计患者一般资料，按照心血管损伤发生情况将其分为A组（合并心血管损伤，84例）、B组（未合并心血管损伤，168例），采用多因素Logistic回归分析法分析类风湿关节炎合并心血管损伤的独立影响因素。结果：单因素分析结果显示，类风湿关节炎合并心血管损伤与体质量指数（BMI）、骨折病史、高同型半胱氨酸血症史、甲状腺疾病史、总胆固醇（TC）、C反应蛋白（CRP）、血小板（PLT）、类风湿关节炎疾病活动性评分系统（DAS28）评分、关节外脏器受累数、类风湿因子（RF）有明显相关性（P＜0.05）。多因素Logistic回归分析显示，BMI（OR=4.194，95%CI=1.845～9.584）、骨折病史（OR=3.236，95%CI=1.225～8.536）、高同型半胱氨酸血症史（OR=6.015，95%CI=1.509～23.978）、甲状腺疾病史（OR=13.564，95%CI=3.218～55.423）、TC（OR=1.352，95%CI=1.312～1.384）、CRP（OR=1.702，95%CI=1.305～2.231）、PLT（OR=1.168，95%CI=1.075～1.274）、DAS28评分（OR=5.035，95%CI=1.629～8.572）、关节外脏器受累数（OR=1.198，95%CI=1.140～1.257）、RF（OR=2.513，95%CI=2.244～2.854）是类风湿关节炎合并心血管损伤的独立影响因素（P＜0.05）。结论：低BMI、伴骨折病史、TC升高、CRP升高、PLT增加、DAS28评分升高、关节外脏器受累数较多、RF阳性均与类风湿关节炎合并心血管损伤密切相关，临床上应引起足够重视。
