摘 要 〖HT5K〗目的:观察肠内高蛋白营养支持治疗对头颈癌患者股前外侧皮瓣修复术后的营养状况与治疗效果。方法:选取头颈癌患者47例,将其随机分为对照组(n=21)与试验组(n=26)。在接受常规治疗干预外,试验组选用流食+蛋白粉\[2.0g/(kg·d)\]进行肠内营养支持,对照组仅自备流食。若出现胃肠道症状,则辅以活性益生菌进行胃肠道调理,要求两组患者每日摄食量相同。记录两组患者术后当天、术后第10天的营养指标(TP、ALB、PA)及免疫指标(WBC、LYM、NEU)差异;及术后腹泻、腹胀及切口感染的发生率。结果:两组患者入院时、术后当日各指标接近,利于对照研究。术后第10天,试验组病例TP、ALB、PA较对照组有明显升高(P<0.01);试验组WBC、LYM、NEU数量较对照组更快恢复至正常水平(P<0.05);试验组术后腹泻、腹胀及伤口感染发生率较对照组明显降低(P<0.05)。结论:流质饮食+高水平蛋白粉进行鼻饲喂养,可明显改善头颈癌患者术后营养状况,降低腹泻、腹胀及切口感染的发生率。
Abstract:
Abstract 〖WT5BZ〗〖HT5”SS〗Objective:to observe the nutritional status and therapeutic effect of enteral highintaking protein powder nutritional support therapy on patients with head and neck cancer after anterolateral femoral flap repaired. Methods:47 patients with head and neck cancer were randomly divided into the control group (n=21) and the test group (n=26).In addition to receiving conventional treatment and nursing,the test group used liquid food + protein powder\[2.0g/(kg·d)\] for enteral nutrition support,while the control group only prepared liquid food,requiring the two groups to eat the same amount of food every day.Serum protein content (TP,ALB,PA) and blood cell count (WBC,LYM,NEU) of the two groups were recorded on the day after surgery and the 10th day after surgery.Postoperative diarrhea,abdominal distension and incisional infection. Results:the indexes of the two groups were close to each other on the day after the operation,which was in line with the control principle and conducive to the comparison.On the 10th day after surgery,TP,ALB and PA in the test group were significantly higher than those in the control group (P<0.01).The number of WBC,LYM and NEU in the test group recovered to the normal level faster than that in the control group (P<0.05).The incidence of postoperative diarrhea,abdominal distension and wound infection in the test group was significantly lower than that in the control group (P<0.05).Conclusion:Highintaking protein powder combined with liquid diet can significantly improve the perioperative nutritional status of patients with head and neck cancer,and reduce the incidence of diarrhea,abdominal distension and incision infection.
