摘要:
摘 要 目的:探讨高强度间歇性运动联合药物治疗溃疡性结肠炎(UC)的临床效果及运动对患者血清促炎因子、免疫功能指标的影响。方法:按照随机抽取方式将符合标准的104例UC患者均分为两组。对照组则单纯接受药物治疗;观察组则在用药治疗的同时进行规律性的高强度间歇性运动干预。对比两组临床治疗效果。测定两组治疗前后促炎因子-α(TNF-α、IL-1β、IL-6)和免疫功能指标水平(Th1、Th2、CD3+、CD4+、CD8+、IgA、IgG、补体C3和C4)。结果:经Kruskal Wallis检验分析,轻度、中度、重度患者临床治疗效果间比较差异均具有统计学意义(均P<0.05);经Mann-Whitney检验分析,两组轻度、中度患者临床治疗效果和总效果间比较差异存在统计学意义(均P<0.05),观察组显著优于对照组;但两组重度患者临床疗效间比较差异无统计学意义(P>0.05)。治疗后,两组促炎因子肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)水平和细胞免疫指标Th1、Th2、CD8+、补体C3、C4水平均明显下降,CD3+百分比、CD4+/CD8+值及IgA、IgG水平均明显升高,且观察组各因子水平明显优于对照组(P<0.05)。结论:高强度间歇性运动联合药物治疗UC可获得更好的临床治疗效果,尤其是对轻中度患者病情治疗效果更佳,这可能与其下调机体炎症因子表达、调节机体免疫平衡、增强免疫力等作用有关。
Abstract:
Abstract Objective:To explore the clinical effect of high-intensity intermittent exercise combined with drugs in the treatment of ulcerative colitis (UC) and the effect of exercise on serum proinflammatory factors and immune function indicators.Methods:104 patients with UC who met the criteria were divided into two groups by a completely randomized method.The control group was treated with drugsalone;the observation group performed regular high-intensity intermittent exercise intervention on the basis of medication.The clinical effects was compared between the two groups. The levels of pro-inflammatory factors (TNF-α,IL-1β,IL-6) and immune function (Th1,Th2,CD3+,CD4+,CD8+,IgA,IgG,complement C3 and C4) were measured before and after treatment.Results:According to Kruskal Wallis test,the differences in clinical treatment between mild,moderate and severe patients were statistically significant (P<0.05).According to the Mann-Whitney test,the difference between the clinical treatment effect and the total effect of the mild and moderate patients was statistically significant (P<0.05),that of the observation group was significantly better than the control group.However,there was no significant difference in clinical efficacy between the two groups (P>0.05).After treatment,the levels of pro-inflammatory factors TNF-α,IL-1β,IL-6 and cellular immune indicators Th1,Th2,CD8+,complement C3,C4 decreased significantly,andthe level CD3+ percentage,CD4+/CD8+ value and IgA,IgG significantly increased,and the levels of each factor in the observation group were significantly better than those in the control group (P<0.05).Conclusion:High-intensity intermittent exercise combined with drug therapy for UC can achieve better clinical treatment effect,especially for patients with mild to moderate disease,which may be related to the down-regulation of inflammatory cytokine expression,regulation of immune balance,and enhancement of immunity.
