摘 要 目的:掌握Wernicke脑病治疗使用维生素B1用法,以期为临床规范使用维生素B1注射液给药途径提供参考。方法:以Wernicke脑病和维生素B1为关键词对知网上的国内文献2014年至今出版的25篇文献进行分析总结，从其给药途径、使用剂量、使用时机和不良反应方面进行分析，总结其使用方法。结果:外文文献都是用硫胺素(维生素B1）静脉注射治疗Wernicke；有8篇中文文献wienicke脑病的治疗是有静脉注射的描述，但对其不良反应没有描述。有关的专业书籍认为wernicke脑病静脉输注维生素B1优于肌内注射；中文文献认为治疗上以先期肌肉注射的最多，并逐渐降阶梯为肌肉或口服给药；有关不良反应的文献认为其过敏反应也可能与药物剂量效应有关。结论：维生素B1注射液超说明书用药是需征得药事管理与药物治疗学委员会的审批同意后再使用，也可以多种给药途径使用，但用前需做皮试，用时密切观察患者变化，并注意鉴别药物不良反应。
Abstract:
Absrtact Objective: To master the usage of vitamin B1 in the treatment of Wernicke encephalopathy, so as to provide reference for the clinical standardization of the route of administration of vitamin B1 injection. Methods: With Wernicke's encephalopathy and vitamin B1 as keywords, 25 articles published in the domestic literature of known from 2014 to now were analyzed and summarized. The route of administration, dosage, timing and adverse reactions were analyzed, and the attention was summarized. Results: In foreign literature, intravenous thiamine (vitamin B1) is used to treat Wernicke. There were 8 Chinese literatures about the treatment of Wienicke encephalopathy with intravenous injection, but the adverse reactions were not described. Relevant professional books consider that intravenous infusion of vitamin B1 is superior to intramuscular injection in Wernicke encephalopathy; Chinese literatures believe that the most treatment is intramuscular injection in advance, and gradually downgraded to intramuscular or oral administration; literature on adverse reactions suggests that its allergic reaction may also be related to drug dose effect. Conclusion: The use of vitamin B1 injection beyond the instructions requires the approval of the Pharmaceutical Administration and Pharmaceutical Committee, and can also be used in a variety of ways of administration. However, skin tests should be done before use, changes of patients should be closely observed and adverse drug reactions should be identified.
