摘 要 目的：分析慢性心力衰竭（CHF）患者血清胱抑素Ｃ（ Cys C）水平与抑郁障碍的相关性。方法：纳入慢性心力衰竭住院患者336例，根据Cys C水平分为Cys C异常组和Cys C正常组，采用汉密尔顿抑郁量表（HAMD）评估抑郁障碍，并测定相关临床及生化指标。对上述患者进行Cys C水平与抑郁评分Pearson相关分析，并对抑郁相关因素进行多因素分析。结果：Cys C异常组抑郁人数比例高于Cys C正常组（P＜0.001）；抑郁评分与Cys C异常水平Pearson相关分析显示：抑郁评分与Cys C异常水平呈正相关（r=0.269，P＜0.001）；Logistic回归分析显示：经过对体重指数、血脂水平、聚集人口、高血压、血红蛋白、白蛋白、B型脑钠肽、糖化血红蛋白的校正后Cys C异常（OR=20.077, P＜0.001）,是抑郁障碍患者的相关因素。结论：Cys C异常可作为慢性心力衰竭患者抑郁障碍的标记物。
Abstract:
Abstract Objective:To analyze the association between cystatin C and depression in patients with chronic heart failure.Methods:336 cases of CHF hospitalized patients were divided into Cys C abnormal group and Cys C normal group according to Cys C level. Hamilton depression scale (HAMD) was used to evaluate depression and the related clinical and biochemical indicators were measured. Pearson correlation analysis was performed to examine the correlation between Cys C levels and depression scores in these patients, and logistic regression analysis was performed to detect depressionrelated factors. Results：The proportion of depression in Cys C abnormal group was higher than that in Cys C normal group (P＜0.001). Pearson correlation analysis between depression scores and Cys C abnormal levels showed that depression scores were positively correlated with Cys C abnormalities (r=0.269,P＜0.001).Logistic regression analysis showed that the Cys C abnormality (OR= 20.077, P＜0.001) was associated with depression after correction of body mass index, lipid levels, aggregation population, hypertension, hemoglobin, albumin, Btype brain natriuretic peptide, glycosylated hemoglobin. Conclusion:Cys C abnormalities are markers of depressive disorder in CHF patients.
