摘要:
摘 要 目的:分析雌、孕激素受体(ER、PR)在局部晚期乳腺癌中的表达及与患者预后相关性。方法:收集晚期乳腺癌患者96例,均经镜下取癌组织进行活检;另随机抽取46例患者进行癌旁组织采集,采用免疫组织化SP法检测两组ER、PR,并分析其表达水平与乳腺癌疾病的预后的关系,采用单因素和多因素Logistic回归分析ER、PR表达水平对晚期乳腺癌患者预后相关危险因素。结果:①癌组织中ER(-)与PR(-)例数均明显少于癌旁组织,癌组织中ER(+)、ER()、ER()、PR(+)例数均明显高于癌旁组织(P<005)。②ER(+)者第1年存活率为4791%,明显低于ER(-)5833%,PR(+)者2年存活率为3645%,明显低于PR(-)5104%,差异具有统计学意义(P<005)。③非条件单因素Logistic回归分析结果显示,年龄、肿瘤直径、ER与PR表达水平是影响晚期乳腺癌患者预后死亡单因素(P<005)。④经非条件多因素Logistic回归模型分析结果显示,肿瘤直径≥2、ER(+)、PR(+)是影响患者预后死亡的独立危险因素(P<0001)。结论:ER、PR在局部晚期乳腺癌患者癌组织中表达水平较癌旁组织高,ER、PR表达水平与肿瘤直径是影响局部晚期乳腺癌患者预后的危险因素,ER、PR表达阳性者病死率较高。
Abstract:
Abstract Objective:To analyze the expression of estrogen and progesterone receptor (ER,PR) in patients with locally advanced breast cancerMethods:Clinical data of 96 patients with locally advanced breast cancer treated were retrospectively analyzedFortysix patients were randomly selected for paracancerous tissue collectionImmunohistochemical SP method was used to detect ER and PR in the two groups,and the relationship between their expression level and the prognosis of breast cancer disease was analyzedResults:①The number of ER (-) and PR (-) in cancer tissue was significantly lower than that in paracancerous tissue,and the number of ER (+),ER (),ER (),and PR (+) in cancer tissue was significantly higher than that in paracancerous tissue (P<005)②The 1year survival rate of ER (+) was 4791%,significantly lower than ER (-) 5833%,and the 2year survival rate of PR (+) was 3645%,significantly lower than PR (-) 5104%,with statistically significant difference (P<005)③Nonconditional univariate Logistic regression analysis showed that age,tumor diameter,ER and PR expression levels were the single factors influencing the prognosis and death of patients with advanced breast cancer (P<005)④Analysis results of nonconditional multivariate logistic regression model showed that tumor diameter ≥ 2,ER (+) and PR (+) were independent risk factors influencing the prognosis and death of patients (P<0.001).Conclusion:The expression level of ER and PR in cancer tissues of locally advanced breast cancer patients is higher than that of adjacent tissuesThe expression level of ER and PR and tumor diameter are the risk factors affecting the prognosis of locally advanced breast cancer patients
