摘 要 目的：分析右美托咪定复合全麻对急性脑出血患者脑保护作用。方法：选取急性脑出血患者80例，按照随机数字法分为对照组（n=40）和观察组（n=40）。观察组诱导后给予静脉注射右美托咪定，对照组诱导后给予等量生理盐水，比较两组给药前（T0）、麻醉诱导时（T1）、全麻插管后5min（T2）、手术中60min（T3）、术毕带管恢复自主呼吸时（T4）的血流动力学水平\[舒张压（DPB）、收缩压（SBP）、心率（HR）\]、血浆S100β蛋白、神经元特异性烯醇化酶（NSE）水平及并发症发生率。结果：两组T0时DBP、SBP、HR水平比较均无明显差异（P＞005），观察组T1、T2、T3、T4时DBP、SBP、HR水平均显著低于对照组（P＜005）；两组T0时血浆S100β蛋白和NSE水平比较均无统计学差异（P＞005），观察组T1、T2、T3、T4时血浆S100β蛋白和NSE水平均显著低于对照组（P＜005）；观察组谵妄、躁动发生率分别为1500％、1000％，显著低于对照组的3500％和3000％（P＜005）。结论：右美托咪定复合全麻能够稳定急性脑出血患者血流动力学水平，降低血浆S100β蛋白和NSE水平，有助于减轻脑损伤。
Abstract:
Abstract Objective：To analyze cerebral protection effect of dexmedetomidine combined with general anesthesia on patients with acute cerebral hemorrhageMethods:80 patients with acute cerebral hemorrhage were enrolledThey were divided into control group(n=40) and observation group(n=40) according to random number table methodAfter induction,observation group was given intravenous injection of dexmedetomidine,while control group was given the same amount of normal salineBefore administration(T0),at anesthesia induction(T1),at 5 minutes after general anesthesia intubation(T2),at 60 minutes after start of surgery(T3),at spontaneous respiration time after end of surgery(T4),haemodynamics levels\[diastolic blood pressure(DPB),systolic blood pressure(SBP),heart rate(HR)\],levels of plasma S100β protein and neuronspecific enolase(NSE),and incidence of complications were compared between the two groupsResults:There was no significant difference in DBP,SBP or HR level between the two groups at T0(P＞005)The levels of DBP,SBP and HR in observation group were significantly lower than those in control group at T1,T2,T3 and T4(P＜005)There was no significant difference in plasma S100β protein or NSE level between the two groups at T0(P＞005)The levels of plasma S100 βprotein and NSE in observation group were significantly lower than those in control group at T1,T2,T3 and T4(P＜005)The incidence rates of delirium and restlessness in observation group were 1500% and 1000%,which were significantly lower than those in control group(3500%,3000%)(P＜005)Conclusion:Dexmedetomidine combined with general anesthesia can stabilize hemodynamics of patients with acute cerebral hemorrhage,reduce levels of plasma S100β protein and NSE,and help to reduce brain damage
